Have you had a new knee or hip joint? Are you struggling with your post-op rehab? Do you need some advice and guidance about your progress?

Sometimes, a couple of sessions of physio can make all the difference in the outcome of a joint replacement, not just with exercises but using hands-on treatments as well to improve joint mobility and relieve muscle tightness. You can expect to see progress following joint replacement for up to 1 year post-op.

Hoffa’s fat pad (infrapatellar fat pad) is, as it sounds, a pad of fat that sits below the kneecap and behind the patella tendon. It is squeezed into a small space which makes it vulnerable to getting pinched and squished.

The fat pad can become irritated by trauma such as a direct blow or fall onto the knee, or by chronic micro-trauma (usually due to the way a person stands, muscle weakness or lack of muscle control as the knee straightens) that, over time causes inflammation.

If you imagine the structure of the fat pad is similar to a bath sponge, when it becomes inflamed, it’s like a bath sponge dunked in water and engorged with fluid resulting in an enlarged inflamed pad that is squished into a small space. Unfortunately, it is also one of the most pain sensitive structures in the knee, so any problem leads to a swollen and very painful knee that dislikes standing or being straight. Hoffa’s fat pad syndrome can really impact upon an individual's daily life.
Treatment and management for this syndrome is physiotherapy with the following aims:
• To settle down inflammation
• To offload the knee
• To correct the underlying cause
