Boultham Park Medical Practice GP Practice Patients who visit a GP for a musculoskeletal (MSK) problem will instead have an appointment with an FCP physiotherapist.

The doctors and staff at Boultham Park Medical Practice are proud to offer the highest standard of patient-centred healthcare; this was recognised back in 2011 when the surgery was awarded the Quality Practice Award by the Royal College of Practitioners. The quality of the service we provide was further confirmed following an Inspection by the Care Quality Commission in May 2016 when we were graded good overall and in each individual area of the inspection. We provide a wide range of Primary Care service, typical of what you should expect from all GP surgeries, and run many clinics for the management of Chronic Diseases such as Asthma, COPD and Diabetes. We have other local services such as the Midwife and a Mental Health Practitioner working out of the surgery and we work closely with our health colleagues across the city. We also have a First Contact Practitioner (FCP) who is a physiotherapist professionally qualified to treat patients without a referral from a GP or other healthcare professional. Primary Care Networks have recently been introduced to help primary care providers to work more collaboratively and at scale. We are part of the Apex Primary Care Network which consists of 5 local GP Practices with a collective patient list size of approximately 52000 patients. We are also part of the Optimus Group of GP practices. These are other GP surgeries in our locality with whom we work closely across a number of areas; in particular in offering improved access for appointments outside of our normal core hours. Boultham Park Medical Practice is also a Training Practice. This involves training the doctors and nurses who have chosen to specialise in primary care and who we all will need to look after us in the future!

To ensure you receive the care and support you’re entitled to, make sure your GP practice is aware of your Veteran status and it is correctly recorded on your medical records.

Why is this important?
If your condition is linked to your military service, your referral can be marked accordingly — ensuring your care is prioritised where appropriate. This does NOT mean priority GP appointments.

* Confidential consultation: Pharmacists will conduct a private consultation to assess your health and suitability for the pill.
* Free service: The service is free of charge, and you can receive either a new prescription or continue an existing one.
Health Checks: the pharmacist may check height, weight and blood pressure, and discuss possible side effects.

There is a recognition that demand in primary care has never been higher, with demand outstripping the capacity of GP surgeries to provide safe and effective care.
There are changes proposed by the NHS to try and make access to care and treatment easier and fairer for patients.
Clinical triage has been identified to help streamline workflows and provide the best possible service to our patients.

Triage is a process of providing a preliminary assessment of the information provided to the surgery to help with care navigation.

Triage can reduce waiting times for appointments
Triage can allow patients to use digital tools to send queries without having to attend the surgery or wait on the phone.
Triage can allow those whom are most in need of our services to be identified and treated promptly.
Triage can help patients plan their appointments to a time that may suit their schedule.
