πŸ“£ Transition Reviews – Has It Been Done?

We’ve been made aware that some children and young people with EHCPs may have been missed when it comes to their transition reviews β€” especially at key stages such as:

➑️ Nursery to Reception
➑️ Year 6 to Year 7
➑️ Year 11 to Sixth Form or College

These are significant milestones. A transition review isn’t just a meeting β€” it’s a vital opportunity to plan properly, ensure the right provision is in place, and make sure the next setting is fully prepared.

If your child or young person is at one of these transition points, we want to know β€” has it been done?

βœ”οΈ Were you meaningfully involved?
βœ”οΈ Is the receiving setting fully prepared?
βœ”οΈ Has the process worked well β€” or have there been challenges?

We want to hear all experiences β€” good and not so good. Your voice helps us understand what is happening locally and ensures no child or young person slips through the net.

I am looking to understand How Physical Education Affects Students with Physical Disabilities and Other Disabilities in Primary and Secondary Education in the Liverpool City Region

This short survey is for parents and carers of children and young people with physical disabilities and/or additional needs who attend primary or secondary school in the Liverpool City Region.
οΏ½We want to understand how inclusive Physical Education (PE) is in schools, what works well, and where improvements are needed. Your responses will help inform future inclusive activity, training and funding applications aimed at improving access to sport and physical activity for all young people.
οΏ½The survey should take around 5–10 minutes to complete.
