The “broken bone theory” usually refers to a way of understanding psychological trauma by comparing it to a physical broken bone. It isn’t a formal scientific theory in itself, but rather a metaphor or teaching tool used in counseling, trauma education, and DBT/CBT-style psychoeducation.

The idea is:

A broken bone needs care, time, and the right conditions to heal. If you keep using the injured limb without treatment, it won’t heal properly and might even get worse.

Similarly, emotional or psychological trauma requires care and the right support. If ignored, minimized, or mistreated (for example, by pushing through without rest or trying to numb it), the trauma can “heal wrong,” leading to ongoing pain or maladaptive coping patterns.

Just like a bone can be re-broken and set properly if it healed in the wrong position, emotional wounds may sometimes need to be reopened and processed safely in therapy so that real healing can occur.

And just as no one would blame someone for having a broken leg, the theory helps reduce stigma: trauma and emotional wounds are not signs of weakness, but injuries that deserve compassion and treatment.

Some therapists also expand it further, saying that you wouldn’t tell someone with a broken bone to “just get over it” or “walk it off,” so we shouldn’t say the same to someone with deep emotional pain.

Many people assume that triggering memories only results from major traumatic events. However, it's often the gradual build-up of minor emotional strains, daily stressors, and unresolved physical tension that quietly accumulates in the body over time. In an effort to protect us, the body instinctively holds onto that energy, even creating a "short circuit" to the brain's threat state. When your brain feels a threat, valid or not in that moment, it dials down your higher order thinking abilities. Left unaddressed, this stored shock from unresolved past trauma can eventually show up as:

Ongoing muscle tightness

Deep exhaustion

Feeling emotionally disconnected

Unexplained aches and pains

Racing thoughts

Mental unrest

Ruminating thoughts

Lingering fear or anxiety

Reliance on artificial sources of stimulation (like social media, caffeine, or substances)

Fear of abandoned

Unstable relationships

Identify crisis

Patterns of self-sabotage

Difficulty making progress or feeling stuck

Feeling displaced

Feeling unable to continue

Urges towards self harm... and more ...

I often get asked which of the many DBT skills and strategies my clients should use in a given situation. I'll pause here to find gratitude that we get to point in our work through the DBT manual that we
A. Realize we are in a autonomic system crisis
B. Realize that our behavioral choices need to be helpful, instead of harmful
C. That our biochemistry is afflicted by the brain's threat system and a strategy is necessary immediately
D. That DBT has given us a variety of options to employ with immediate effect
E. That it's going to work straight away, even just a little bit, making us more effective in managing the hard times.

My go-to answer for any suggestions on "which skill" is always the same....
