Ensure Staff Are Properly Trained and Competent

Verify that all staff involved in PEG care have up-to-date, role-appropriate training.

Maintain clear competency assessments and refresher schedules.

Promote a Culture of Curiosity and Escalation

Encourage staff to ask questions and report concerns early.

Reinforce that changes in a client’s condition should never be ignored or “worked around.”

Common Mistakes in PEG Management

Understanding common errors helps prevent harm and improve outcomes for clients with complex care needs.

1. Poor Hand Hygiene and Infection Control

Not washing hands correctly before and after PEG care

Inconsistent use of gloves or PPE
Risk: Infection at the stoma site or systemic infection

2. Inadequate Stoma Site Care

Failing to clean and dry the site daily (or as per care plan)

Not checking for redness, swelling, discharge, or pain
Risk: Skin breakdown and infection

3. Incorrect Tube Position Checks

Assuming the PEG is in place without checking the length or fixation

Ignoring changes in external tube markings
Risk: Feeding into the wrong position or tube displacement
