Hope for premature births

The Guardian recently reported on a pioneering project from Eindhoven University of Technology is exploring a radical idea: a device to transfer a premature baby from a natural womb to an artificial womb. Juliette van Haren and Frank Delbressine are developing this transfer system to bridge the gap between birth and the intensive care needed in an artificial womb, a concept that could redefine neonatal care and survival prospects for extremely preterm infants. Can Twins Show on a 12-Week Ultrasound Scan?

If you’re pregnant and wonder whether a 12-week ultrasound can reveal twins, the answer isn’t always clear-cut. A 12-week scan can sometimes detect twins, but not all twin pregnancies are identified this early. The baby's position, amniotic fluid, ultrasound quality and the technician’s experience can affect detection. Some twins may be seen later in the first trimester.

Understanding twins helps set expectations. Identical twins come from one fertilised egg that splits and usually share one placenta, though they may have separate sacs. Fraternal twins come from two eggs and two s***m, with their own placenta and sacs. The ultrasound can sometimes distinguish these scenarios, but may require follow-up imaging.

If you don’t see a second baby at 12 weeks, don’t panic as many twin pregnancies are confirmed later. Conversely, a single baby at 12 weeks doesn’t guarantee there won’t be twins identified later. Your healthcare obstetrician will guide you on what the images mean and whether additional imaging is needed.

Is It Safe to Take Paracetamol During Pregnancy?

There has been much debate in the US over the last few weeks about the use paracetamol (acetaminophen) in pregnancy. It’s important to note that these discussions do not change the UK position.

In the UK, Paracetamol is considered safe in pregnancy when used at the recommended dose and duration. It is important however that you do not exceed the prescribed dose or use multiple products that contain paracetamol.

We also strongly recommend that for any ongoing concerns, pain or fever, if you are over 6 months pregnant or if you have other health conditions or are taking other medicines, you talk to your GP as a matter of urgency.

For more information on EU/UK context guidance, see the European Medicines Agency statement: https://www.ema.europa.eu/en/news/use-paracetamol-during-pregnancy-unchanged-eu

Reasons IVF May Not Work and What to Do Next

If you’ve been through IVF or are considering it, it’s important to know that a successful outcome isn’t guaranteed, and there are several reasons why an IVF cycle might not work. Sometimes the factors are medical, such as poor embryo quality, chromosomal abnormalities, or other underlying health conditions that affect implantation or early pregnancy. Other times response to stimulation isn’t optimal, leading to fewer and/or poorer-quality eggs. Age, previous pregnancies and certain fertility diagnoses can also influence success rates.

Experiencing a failed cycle can be incredibly tough, but there are constructive steps you can take moving forward. First, speak with your fertility specialist to understand what may have limited success in your specific case. Ask about the quality and fate of embryos, any potential uterine or endometrial factors and whether there are adjustments to the stimulation protocol or lab techniques that could improve outcomes in future cycles.

We always recommend a second opinion if you’re unsure about the recommendations you’ve received. It can also be helpful to discuss lifestyle factors that might support fertility such as maintaining a healthy weight, managing stress, avoiding smoking and ensuring you’re taking any recommended supplements or medications as directed.

Beyond medical considerations, emotional support is essential. IVF can be emotionally demanding, so seeking support from trusted friends, family, or a fertility counsellor can help you navigate the rollercoaster of emotions and decision-making.

Remembering Jean Purdy, the IVF Pioneer

Jean Purdy was a key figure in the development of in vitro fertilisation, working alongside Robert Edwards and Patrick Steptoe. She contributed extensively to the science, care and meticulous record-keeping that helped turn IVF from a dream into a clinical reality. Purdy’s work underpinned the early breakthroughs that gave hope to countless families struggling with infertility, and her dedication and scientific rigour helped ensure that IVF progressed with both compassion and precision.

Understanding Vaginismus and What Causes It?

Vaginismus occurs when the muscles around the va**na tighten involuntarily, which can make pe*******on painful or impossible. The causes are often a mix of physical, psychological, and emotional factors which can vary from person to person. Common contributors include past painful medical experiences or va**nal infections that create fear of pain, anxiety or stress related to s*x or intimacy that build up tension, relationship issues or lack of trust with a partner, or a history of s*xual trauma can influence the muscle response and pain. Medical conditions such as infections, va**nal dryness, hormonal changes, endometriosis, or pelvic floor disorders can also make in*******se uncomfortable.

Often vaginismus results from multiple factors rather than a single cause, and it’s possible to have a combination of psychological and physical influences. If you’re seeking answers, talking with a healthcare professional who specialises in s*xual health or pelvic floor disorders can help identify potential physical causes.

Treatment is typically multidisciplinary, addressing physical muscle relaxation training, psychotherapy, and guided gradual exposure to reduce fear and improve comfort with s*xual activity.

What is Loop Excision (LEEP)?

Loop Electrosurgical Excision Procedure, or LEEP, is a minimally invasive treatment that uses a thin, circular wire (loop) with electric current to remove abnormal or precancerous cells from the cervix. It’s commonly used to treat early cervical changes detected on screening tests.

It’s typically recommended after cervical abnormalities are found on a cervical screening, especially when there are signs of precancerous changes or cervical dysplasia. The loop gently removes the abnormal tissue, and the area then heals over time. Your doctor will monitor your cervix afterward to ensure everything looks as it should be.

Benefits include a quick in-office procedure (often a same-day treatment), targeting only the abnormal tissue to preserve healthy tissue, and it can be done with local anaesthesia.

After the procedure, you may experience some light bleeding or discharge for a few days, temporary pelvic cramping or tenderness and follow-up visits to confirm healing and check for any remaining abnormal cells.

When should you have a s***m test?

Deciding whether a couple should have a s***m analysis is a common step on the path to understanding fertility challenges. Semen analysis can provide valuable insights into male fertility including s***m count which helps your healthcare team tailor next steps. It’s important to know that fertility is a shared journey and having an open conversation with both partners, as well as your clinician, can reduce uncertainty and guide you toward the most effective plan.

A semen analysis is usually recommended after a period of trying to conceive without success, and it can be part of a broader fertility assessment. Your clinician will discuss what the test involves, how to prepare, and how to interpret the results in the context of both partners’ health, lifestyle and fertility goals. It’s also worth noting that a single test doesn’t tell the full story; sometimes multiple samples are needed to get an accurate picture.

If you’d like to learn more from a trusted medical resource, the Cleveland Clinic offers clear information on semen analysis here: https://my.clevelandclinic.org/health/diagnostics/21520-semen-analysis
