Managing S1 Dysfunction requires a precise balance of mobility and stability. Whether you are dealing with sciatica or SI joint discomfort, these four foundational exercises focus on neural desensitization and lumbo pelvic control.

​Rehabilitation Essentials for S1 Dysfunction

​1. Nerve Gliding (Sciatic Nerve Mobilization)

​The Goal: Improve neural excursion and reduce tension.

​Key Tip: Keep movements gentle. Do not push into sharp pain; we want to "floss" the nerve, not stretch it aggressively.

​2. SI Joint Stability (Piriformis Stretch)

​The Goal: Alleviate compression on the sciatic nerve.

​Key Tip: Maintain a neutral spine while pulling the knee toward the opposite shoulder.

​3. Glut Max & SI Joint Strength (Glute Bridges)

​The Goal: Activate the gluteus maximus to stabilize the pelvis.

​Key Tip: Squeeze at the top of the bridge and ensure the movement comes from the hips, not the lower back.

​4. Core & Pelvic Stability (Bird Dog)

​The Goal: Strengthen deep core stabilizers for essential lumbar support.

​Key Tip: Focus on "anti-rotation." Keep your hips level as if there is a glass of water on your lower back.

​Clinical Note: Always assess the patient's irritability levels before prescribing. Modifications are key to long-term recovery.

Don’t ignore these signs in feet while doing your physio assessment.
Remove the shoes and socks to get a full picture.
Observing is not enough!!Palpate, touch, feel— check the pulse.

Take special care of diabetic patients and watch out for signs of neuropathy.

Skin temperature, discolouration are big indicators of underlying vascular impairments. This could eventually lead to lack of nutrition to the nerves causing neural death.

Dry flaky skin of the feet is another indicator of circulatory impairment. Ask about type of pain in the feet to identify nerve pain, joint pain or muscle pain

And for heavens sake don’t miss doing shoe evaluation — inside and out!!
