It was lovely to catch up with this young lady 2 months after she underwent a piezo rhinoplasty under my care.

She was particularly troubled by excess width from the front and a bump at the side. As is common in my practice, it was very important for her to have a natural outcome.

The key manoeuvre in her case was repositioning of the lower lateral cartilages in the tip. In the before photo, they point upwards and fill out the supratip. During surgery, they were reoriented into a more caudal (towards nostrils) position giving a more defined appearance. See last photo for visual of change in position.

Although my preferred option is to build a little slope from the side, she was keen for a straight bridge - which works very well with her features.

I have just returned from the Face and Rhinoplasty Symposium ( ) in Florida - an intense educational meeting.

The emphasis on the facelift side was deep neck work and endoscopic approaches with the maestros and .gomezbravo leading the way. There were some interesting discussions about the goals of facelift surgery - being just rejuvenation or an element of beautification too. With endoscopic approaches, it certainly opens up surgery to a younger cohort.

There were also some valuable discussions on complex rhinoplasty cases .md . I am always looking for tips to refine my technique.

The meeting finished with an endoscopic cadaveric course. It was helpful to pick up tips on how we can improve our own cadaveric next year in Cambridge, which is now in its 4th year 🥳 . I am also aiming to introduce endoscopic facial rejuvenation approaches in my own practice.

I always put simulations and patient photos up on the wall during surgery so that I can make sure we are progressing accurately towards the end goal. Some patients also bring their own goal photos to put up - creating a mood board!

There is a long way to go in the recovery process but it was lovely to see the change and Laurie’s positive reaction at this stage.

I am thrilled to share an update on this lovely 54-year old patient’s facelift journey. We are now one year post-op and it was fantastic to see how well she is getting on - feeling confident and looking the young age she feels. In fact, it was amusing to hear how many times her date of birth has been questioned over the last few months on various travel trips.

A year ago we performed a combined deep plane facelift, deep neck reduction and volume restoration with fat grafting. Unfortunately the pre-op pictures are not optimised light-wise to see all the contours and folds fully - but nonetheless, you can still appreciate the magnitude of change.

This young lady was troubled by excess height in the nose that created an imbalance in facial proportions. An ultrasonic rhinoplasty technique was used to reduce the height of the bridge and also de-rotate the tip to create a natural and feminine profile. She was thrilled with the change when we caught up in clinic last week - 1 Year on from surgery.

Deprojection of the nose (without a bump) is a less common indication for rhinoplasty and best suited for patients with thin or average thickness skin. The trick is to maintain (or improve) tip definition and contour in the front profile despite creating a tissue excess from the height reduction.

I am very pleased to share some of my insights from the IFNS Facial Nerve meeting in Madrid. It was a tremendous gathering of facial paralysis specialists from around the globe, pulling in 650 delegates - plastic surgeons, ENT surgeons, therapists. A truly collaborative meeting, which is an honest reflection of the discipline.

There was a mixture of old and genuinely new concepts, some disagreement and some consensus, some ideas to discard and some that we will hopefully take back to our practice at Guys & St. Thomas’.

So often plastic surgery meetings are presented on social media as a tidal wave of self-congratulatory, self-serving back-slapping images, so it is important to convey the substance not just the show! This meeting was rich in substance. (Of course - a bit of heckling is to be expected when videoing - thank you Greg, Lauren & Sarah for keeping me grounded).

Disappointed by fillers but not quite ready for a facelift?

Here are 3 procedures to consider before taking the plunge into a facelift.

1. Deep neck reduction - either through a short scar technique in younger patients or a full neck lift approach. A youthful neck contour gives confidence from every profile angle.

2. Buccal fat reduction. A simple procedure that helps contour the lower face by reducing volume, particularly beneficial in patients with naturally round faces.

3. Chin and jawline implant. The implant is placed through the same incision as the neck lift (under the chin) and helps restore a defined jawline. It may reduce jowls. Fat grafting is an alternative.

These 3 procedures may be performed individually or in combination to help contour the lower face, jawline and neck.

Rhinoplasty is one of my very favourite procedures - so varied, so many technical intricacies, so challenging but very enjoyable and very rewarding. It has the potential to be completely transformative - make a genuine difference to patients lives, improve confidence and function.

Yesterday, I performed 3 primary rhinoplasties - all completely different.

The first case was a straight profile (no bump), just too too high and out of proportion to her facial features. This is a less common indication but can also be transformative, creating a much more balanced and feminine profile.

The 2nd case (and 2nd photo here) was probably the most challenging - thick skin, bulbous tip,
high septal deviation, short septum - more tricks required to get it right, more pitfalls to be wary of, but very fulfilling and I can’t wait my patients reaction when we remove the cast.

The final case (not shown here) was a classic rhinoplasty nose - trio of bump on bridge, droopy tip and septal deviation.

More than any other facial procedure, the surgical plan needs to be meticulously carefully tailored to the individual.

I am delighted to share these early post-op results 2 months after a short scar neck lift, one of my favourite procedures 😊!

This lovely 30-year old lady was bothered by a full obtuse neck despite being young and petite. This is a common situation I see - completely unrelated to ageing or weight change.

The appearance is symptomatic of natural structure not laxity - low hyoid bone, deep fat herniation, bulky and low submandibular glands, weak platysma. These are common features of patients in their 20s and 30s with a lack of neck contour. It often coexists with a weak chin.

By the time patients come to me, they have often tried out several non surgical treatments with minimal or no benefit (fillers, HiFU, fat-dissolving injections). In fact, these treatments may create problems and make surgery more difficult.

The most consistent solution in my experience is a short scar neck lift - which addresses the underlying structural issues directly.

It is a daycase general anaesthetic of around 2 hours and is performed through a smallish incision under the chin. The risks, recovery and expectations are discussed in detail during the consultation process.

Our Story

I am a Consultant Plastic surgeon in London with expertise in facial aesthetic surgery and complex facial reconstruction including facial palsy. I am a graduate of both Oxford and Cambridge Universities and have trained in many major Plastic Surgery units in London, Oxford and Toronto.

I provide a boutique aesthetic surgery service in London. Rhinoplasty and facelift surgery form the bulk of my practice and revision work forms a major part of my practice. I also enjoy and have a special interest in eyelid and breast reshaping surgery. As well as my cosmetic practice, I run a busy reconstructive practice at Guy’s and St. Thomas’ Hospitals in London where I was Chief of Plastic Surgery from 2016-2019. I have a specialist interest in advanced corrective treatments for patients with facial palsy.
