Let’s talk Thyroid Eye Disease 👁️ Those wide, “staring” eyes that patients often describe pain, watering, light sensitivity, and worsening dry eye because the tear film simply can’t protect the surface properly.

For dysthyroid lid retraction, the treatment is lid lowering surgery, releasing scarred, fibrotic muscle so the eyelid can return to a healthier, protective position.

I use a technique I described years ago: anterior graded lid lowering, where each scarred layer is carefully released. It’s technically demanding and not always predictable — lids can under-drop or over-drop — which is why this remains one of the most challenging procedures in oculoplastics.

But when done well, it can restore comfort, eye protection, and a more natural appearance.
This was one of those complex cases and exactly why specialist care matters.

Lower eyelid ageing isn’t one-size-fits-all and neither is treatment. 👁️✨

When I assess under-eye concerns, I’m not just looking at bags or loose skin, I’m looking at skin quality, volume, and structure.

1️⃣ Traditional lower lid blepharoplasty works best when there are eye bags and hollows, allowing us to reposition or debulk fat and restore contour.
2️⃣ Skin pinch blepharoplasty is ideal when there’s mild excess skin with good skin quality, a precise, minimal approach.
3️⃣ Laser, Tixel or radiofrequency microneedling are better when the issue is thin, crepey skin, not true excess, because quality matters more than quantity.

The right result comes from choosing the right technique, not the most invasive one. Thoughtful assessment always comes first. 👁✨
