Mouth breathing is often overlooked, but it can sometimes indicate that the nose is not functioning as it should.

The nose plays an important role in warming, filtering, and humidifying the air we breathe. When airflow through the nose is restricted, the body may compensate by breathing through the mouth instead.

Persistent mouth breathing can affect sleep quality, throat comfort, and overall respiratory health. In many cases, the underlying cause may be a structural issue within the nose, such as a deviated septum or narrowing of the nasal valves.

As a Consultant ENT and Facial Plastic Surgeon, Mr Florian Bast assesses nasal breathing in detail, helping determine whether symptoms are due to inflammation, congestion, or structural obstruction.

Understanding the cause is the first step towards improving nasal airflow and restoring healthy breathing.

Grateful to my lovely patient for allowing me to share these pre- and post-operative results.

Thick skin does not have to be a disadvantage in rhinoplasty. What matters most is a precise and stable reconstruction of the cartilaginous framework, allowing the new nasal shape to be properly supported and defined.

The procedure was performed using an open structural approach. The columellar incision, as well as the incisions from alar base and nostril sill reduction, heal extremely discreetly and are virtually invisible once matured.

For skin closure, I use very fine absorbable sutures (Vicryl Rapid 7-0), as fine as a human hair, to ensure the most delicate wound healing possible.

Nasal valve collapse is one of the most common causes of persistent nasal breathing difficulty, yet many people are unaware it exists.

The nasal valve is the narrowest part of the airway, meaning even small changes in structure or support can significantly affect airflow. Patients often describe a sensation of blockage, restricted breathing, or the nose collapsing inward when inhaling, particularly during exercise or deep breathing.

Because these issues are not always visible externally, careful assessment of the nasal structure and airflow dynamics is essential.

As a Consultant ENT and Facial Plastic Surgeon, Mr Florian Bast routinely evaluates the nasal valves when investigating breathing concerns and when planning functional or revision rhinoplasty.

Identifying the underlying cause is the first step towards restoring comfortable nasal breathing.

A recent patient shared how surgery with Mr Florian Bast helped address both the appearance of an old nasal injury and long-standing breathing difficulties.

They described the process as smooth and well supported from start to finish, with each stage of treatment clearly explained and regular follow-up care provided throughout recovery. Feeling confident and reassured during the consultation and surgical process was an important part of their experience.

Following the procedure, they reported significant improvement in their breathing for the first time in their life, alongside a straighter nasal profile. They also highlighted the professionalism and supportive approach shown by Mr Bast and the wider medical team.

Feedback like this reflects Mr Bast’s commitment to clear communication, careful surgical planning, and long-term patient care, particularly when treating the functional and aesthetic effects of previous nasal injuries.

Your nose does far more than shape your appearance; it plays an important role in protecting your lungs.

When you breathe through your nose, the air is warmed by a rich network of blood vessels within the nasal passages. In many cases, inhaled air can be warmed to almost body temperature (around 37°C) before it reaches the lungs.

At the same time, the nose also filters particles and humidifies the air, helping create optimal conditions for the respiratory system.
This is one reason why nasal breathing is healthier than mouth breathing, and why structural nasal problems can have a significant impact on overall wellbeing.

As a Consultant ENT and Facial Plastic Surgeon, Mr Florian Bast frequently assesses nasal airflow and structure to help patients achieve healthier breathing.

What makes a rhinoplasty plan truly bespoke?

No two noses are the same, and neither are the goals, anatomy, or functional needs of each patient. A tailored rhinoplasty plan takes into account far more than the external shape of the nose. It requires careful assessment of facial balance, skin thickness, cartilage strength, nasal airflow, and long-term structural support.

For many patients, breathing function is just as important as aesthetics. This is why a thorough consultation and detailed surgical planning are essential before any procedure is considered.

As a Consultant ENT and Facial Plastic Surgeon, Mr Florian Bast approaches rhinoplasty with a focus on both form and function, ensuring each treatment plan is designed around the individual, not a template.

Because the best results are never one-size-fits-all.

Always rewarding to see long-term outcomes like this.

In noses with thick skin and weaker cartilage, achieving a lasting result requires solid structural reconstruction rather than just surface changes.

The patient was particularly bothered by the dorsal hump and the tip plunging when smiling. With piezoelectric precision for the bony work and a strong septal extension graft to control the tip, we were able to create a more balanced and stable result.

Form and function restored, and most importantly, a happy patient.

Thinking about rhinoplasty? There are a few important things to keep in mind before your consultation.

Patients are always encouraged to ask questions and discuss potential risks openly. Understanding the procedure fully is an important part of making an informed decision.

It’s also important to remember that in rhinoplasty, even small adjustments can create significant changes to the overall balance of the face. Careful planning and realistic expectations are key.

Healing is a gradual process, and final results develop over time, often many months after surgery, rather than within the first few weeks.

And if it helps you feel more comfortable, bringing a friend or family member to your consultation can be a great way to support you through the process.

In this Q&A, Mr Florian Bast, Consultant ENT and Facial Plastic Surgeon, shares some key things patients should consider before taking the next step.
