This week I am feeling quite pleased with myself as I’ve helped a new patient who has already seen several other therapists with no improvement, reach a significant diagnosis of Polymyalgia Rheumatica (PMR), an inflammatory condition thought to be autoimmune in origin.

This patient was referred to me having been told his problems were due to ‘tight fascia’ (ahem), but once I’d ascertained that his

* bilateral hip stiffness and pain leaving him struggling to walk, was
* worse in the morning, and
* had a rapid and recent onset
* at the age of 67

I knew he needed blood tests to check his inflammatory markers, highly suspecting PMR (above other inflammatory conditions).

Being an Osteopath means getting involved in all aspects of peoples’ health, not just MSK issues; we are trained in pathology and differential diagnosis so that we know when to refer someone for blood tests for further investigation of MSK issues that are potentially symptoms of systemic health issues, i.e. inflammatory, autoimmune, infectious - plus I did extra training in functional medicine, and further CPD in interpreting blood panels, and I am familiar with many diagnostic pathways required to be followed in order to reach diagnosis.

In this patient’s case, his blood tests (he went private) threw up significantly elevated ESR, CRP and IGa, confirming signs of systemic inflammation - his GP had also recently increased a statin medication recently (which itself can also cause myalgia) so they also as a precaution tested his Creatin levels to rule out statin-related muscle damage, thankfully these were low, and less likely given the elevated inflammatory markers. Based on these results, his GP is likely to try him out on a diagnostic dose of Prednisolone (a steroid medication), which will confirm PMR if his symptoms reduce, then once symptoms reduce, tailor off the medication ideally, or find the lowest dose to manage his symptoms. I’ve advised and supported him every step of the way 🙏

This is where I work on Thursdays! Well, not at Selfridges, and not on Oxford St, but just behind on the bottom of Baker Street, at Light Centre Marylebone, 9am-7pm 💪 But I pass Selfridges every Thursday on the walk from Bond St station, and I just love the art deco fascia, it makes me smile every time! It’s also my shortest journey to work of all my commutes, 5 stops on the Elizabeth Line from home to Bond St, which just makes life easy 😊

Today I cycled to Richmond and back, practicing the commuting route to the Richmond clinic. 22.8 miles round trip 💪 52 mins there. 1 hr 06 mins on the way back (I got lost when Google Maps lost connection, my sense of direction is zero ha!), and in both directions Google directed me to a footbridge that I’d have to carry the bike up and walk it over - nah. But altogether, lots of fun! 🚲 Mostly cycle paths, a few quiet roads. Good bit of exercise. That’ll do. I do like to practise the route to new places, and places to lock the bike up (how amazing is Richmond Cycling Hub at the Station!), before I dive in - my folding e-bike is being retired for campervan duty in Germany, so awaiting the new bike to arrive which hopefully should be in a couple of weeks, then let the cycling commence!

Hello from Marylebone! 👋🙌

These days, I’m now only working in Crystal Palace one day a week, so my diary there is very backed up… next available appointment is 8th April!

So as I keep saying to everyone who I usually see there, come and see me at my other locations 😊

✨ Belgravia ✨on Tuesdays 9am-7pm and Fridays 12-3pm, right next to Victoria Station, the easiest clinic away from Crystal Palace.
✨ Marylebone ✨ on Thursdays 9am-7pm, equally between Bond St, Marble Arch and Baker St stations, so if you work centrally, very easily accessible from many different tube lines.
✨ Richmond ✨ on Mondays 12-6pm - okay so this one is a bit of a stretch for Crystal Palacians, but every time I mention it, people say “oh I love Richmond, I could make a day of it and go for a walk along the river” - yes!! 😊 I have had people who used to see me in Crystal Palace, come to see me in Richmond 🥰
