We’ve had a fantastic day exhibiting at the Devon Care Homes Collaborative Annual Conference today!

It was wonderful to meet so many dedicated care home professionals from across Devon, share ideas, and talk about the important work happening to improve care for residents.

At the Gold Standards Framework (GSF) stand we had some great conversations about how care homes can strengthen proactive, person-centred end-of-life care, and it was inspiring to hear about the commitment and passion across the sector.

What does excellent end of life care look like in a care home setting?

A case study from a GSF Accredited Care Home tells the story of a resident whose needs were recognised early, whose wishes were documented through Advance Care Planning, and who chose to remain in the home for comfort-focused care.

When she could no longer walk by the river she loved, staff found creative ways to help her experience it again. She died peacefully, with family by her side, listening to music she loved.

This is the impact of structured, compassionate planning.
