We provide services for a range of service users from young adults with complex needs, to the elderly requiring personal support and domestic help. We believe everyone should be given every opportunity to be cared for in the comfort of their very own home, that is part of our person-centred approach which always produces the best outcome. At Conquest Care and Support Agency LTD, the care plan of service users are tailored responsively to their wishes and choices. We review, monitor and tailor the care plan to ensure changing needs are met. Our team is experience in managing dementia care, reablement (rehabilitation) and complex continuing care.
