This is an interesting case where the patient has a Hickman line in place for parenteral nutrition. A Hickman line is a soft plastic tube that is placed in the chest and enters within a central vein in the neck.

For patients who have, or may need, a Hickman line, this does not automatically prevent breast surgery. In this case, the procedure was performed by making an incision below the breast and working beneath the muscle, meaning the Hickman line was not affected in any way, we were really far away from it. But it would t stop other types of breast surgery if they were carefully planned.

Catching up with today for her follow up. We talked through her breast surgery journey and what it really felt like from start to finish. The questions, the nerves, the decisions and the recovery are all part of it.

In certain arm lift cases, stopping the incision above the elbow can create a dog ear effect in the proximal forearm, leading to laxity below the incision line.

This patient underwent an extended arm lift, meaning the incision continues past the elbow, to ensure safe removal of excess tissue whilst maintaining healthy blood flow, by measuring the amount of skin excess that we can remove, to avoid a tourniquet effect that would cause potential tissue necrosis in the arm

The result is a smooth, continuous contour along the entire upper arm. Thank you to my wonderful patient who allowed me to share this procedure.

When an implant descends lower than it should, it can distort the overall shape of the breast. This is known as bottoming out, and it often creates the illusion that the ni**le is positioned too high - when in reality, the implant has shifted downward.

For this patient, Mark restored the correct breast position by tightening the lower pole pocket and providing stronger support internally. By re-establishing the proper boundaries of the pocket, the implant sits correctly once again, and the ni**le appears naturally centred without any direct work to the ni**le itself.

This is one of the most common problems with implants. When an implant drops too low - a complication known as bottoming out - it can make the ni**le appear unusually high on the breast.

However, the ni**le isn’t actually the problem - the implant is sitting too low.

In this case, Mark corrected the issue by tightening and reinforcing the lower breast pocket. This immediately re-centralised the ni**le, restored balance, and returned the breast to its proper anatomical position, all without altering the ni**le itself.

thank you to my lovely patient who allowed me to share their pictures! The challenge in this operation was to try avoid skin laxity after the liposuciton which can only be treated with a thigh lift. We discussed this pre-operatively but we thought we could possibly avoid it by adding renuvion therapy, which heats the under surface of the skin and creating a little bit of scar tissue which when it contracts. This is with the use of plasma energy. As you can see the pictures post operatively are about 2 months after the surgery and there is very good skin contraction and there is no need for thigh lift. The patient is really happy because the skin is nice and smooth and the best thing is that it will actually keep on getting better because the scar contracture will take a good 6 to 9 months, so what you see now will only improve. Overall we removed 2 litres of fat from both sides. about a litre from either side, which just from the inner thighs is quite a significant amount. Both patient and surgeon are super happy with the outcome

A really big thank you to my wonderful patient for allowing me to share these pictures with you, since it’s one of the most unusual cases you will see. This is also for fellow plastic surgeons who may not see this case, or may not know how to do this case, because you really need to think outside the box.

So this patient has significant chest asymmetry. The right breast was a lot bigger than the left, and this was due to rib cage asymmetry, so the right side was a lot more protruding. If you just do the traditional thing and put a much bigger implant in one side than the other, this will not really treat the upper part of the breast, as you can see on the video.

So in this case we had to really think outside the box, as I said, and put a small round implant (155 cc’s) on the right side, and a 340 cc anatomical implant (tall height) on the left side. Meaning that it’s more tall than it is wide, and that would fill up the upper part of the breast.

Another thing that you could possibly do in this situation is maybe do some fat transfer in the upper pole and use a round implant, but in her case, as you can see, there is very little fat available. And the operation, the way we did it, worked magnificently. She is over the moon, I am over the moon, and it’s a good case for you to see and for fellow plastic surgeons to learn from.
