๐Ÿค” The Great OT Debate: Sharpening Our Clinical Reasoning and Decision-Making Skills

โ‰๏ธ Is there ever truly one right answer in Occupational Therapy clinical reasoning?

โ‰๏ธ Does every patient require the same management plan based solely on their condition?

โ‰๏ธ What happens when a patientโ€™s wishes conflict with our professional opinion?

โ‰๏ธ And how do we defend our reasoning to members of the MDT who may not fully understand the role of OT?

To celebrate OT Week in 2024 and 2025, Occupational Therapists at Whipps Cross Hospital engaged for the first time in โ€˜Debate Clubโ€™โ€”an innovative addition to our wider OT Club teaching programme. The sessions focus on the nuances of clinical reasoning and decision-making within everyday practice.

๐Ÿ‘‰ Keep an eye out for some of the thought-provoking topics our OTs at Whipps Cross Hospital have been debating.

Our first OT Club training for this year was on an Introduction to Hand Therapy. The session was hosted by our outpatient hand therapy colleagues. We defined hand therapy as a discipline and what the role is for OTs, briefly reviewed anatomy and physiology of the upper limb and hand, and discussed the significance and importance of functional hands for ADL engagement. We briefly looked at some common chronic hand conditions including OA, arthritis, Carpal tunnel, radial nerve palsy (Wrist drop) tendonitis (such as DQT), and reviewed what constitutes as trauma conditions. We covered basic hand therapy assessment and treatment, and how these can best be applied within an acute care inpatient setting, with a focus on the management of stiffness, oedema and pain.

We wanted OTโ€™s to;

โ€ข Understand the importance and role of hand therapy and relate it back to the acute inpatient setting.

โ€ข Understand how to conduct a basic hand therapy assessment.

โ€ข Be able to recommend simple exercises for managing common chronic conditions.

โ€ข Be able to identify and treat common upper limb symptoms prevalent within inpatient settings, such as oedema, stiffness, pain and weakness.

๐—ž๐—ฒ๐˜† ๐˜๐—ฎ๐—ธ๐—ฒ๐—ฎ๐˜„๐—ฎ๐˜†๐˜€:

Staff observed that Occupational Therapists have the potential to play a greater role in supporting common hand therapy conditions on acute wards, enhancing both hand function and prevention of the development of possible longer-term issues.

Following this teaching session therapists felt more confident in their ability to independently identify and manage potential concerns of the upper limb and were more aware of referral processes and supports available via our hand therapy expert colleague.
