Greater Manchester Integrated Care Partnership Improving the health and wellbeing of Greater Manchester. Working to tackle the causes of inequalities, preventing poor health and improving life chances.

Have you heard of Live Well? It is Greater Manchester’s community-powered movement for health and wellbeing, driven by local people with support from public services.
Live Well tackles health inequalities at their roots, shifting power and funding into the hands of communities. It brings people, local charities, councils, the NHS, and faith groups together to co-create healthier, happier lives for all.

We believe carers in Greater Manchester deserve early identification, easier access to healthcare and support to stay in employment or education.
As a carer, knowing your rights empowers you with information. Your local carer’s centre can help you access the support you are entitled to and provide emotional and social support.
We popped into Bolton Carer’s Centre to find out about the support they offer.
You can find details of your local carers centre on our website, where you can also read about our Carers Charter and Carers Commitment. Just follow the link in the comments.

Did you know that Live Well supports the armed forces community? Back in September, colleagues hosted an event exploring Live Well for the Armed Forces Community in Bury. Together with the Office of Veterans Affairs, it was discussed how Live Well spaces and centres could be shaped to meet the needs of the community. Aligned with the development of the National Armed Forces Covenant, the event highlighted the commitment to support those who have served, and the drive to embed this within Live Well movement.
You can see an example of community-led practice by veterans in Denton in this community reporting video about great everyday support in our neighbourhoods.

💛⚽ Supporting Men’s Mental Health — Mind, Body & Connection

Thanks to dedicated men’s mental health funding from Salford City Council and Greater Manchester Integrated Care Partnership , Jak’s World is supporting young men aged 18–25 through football, gym drop-ins, and open conversations about wellbeing. ⚽💪

At Jak’s World, we meet young men where they are — on the football pitch and in the gym — creating spaces to move, talk, and connect. 💛

Our approach reflects the Five Ways to Wellbeing, helping young men:

🤝 Connect through friendship, teamwork, and shared experiences
🏃‍♂️ Be Active through football and gym sessions
🌿 Take Notice through mindfulness, reflection, and emotional awareness
📚 Keep Learning with Su***de First Aid training and practical support
💛 Give through volunteering, fundraising, and supporting others in our community

We’re extremely proud that the world-renowned mindfulness teacher Eckhart Tolle and the Eckhart Tolle Foundation are supporting this project. Young adults connected to Jak’s World can access Embodied Presence: A Journey for the Next Generation — a 7-hour online mindfulness course including never-before-seen footage of Eckhart speaking directly to young people, answering their questions, and sharing practical tools to manage stress, build emotional strength, and develop presence. 🌿

📲 Scan the QR code to request free access and mention you’re from Jak’s World.

Together, we’re building stronger minds, stronger bodies, and stronger communities. 💛

Let’s talk about health care checks!
Hundreds of people in Greater Manchester have avoided life-changing illness in the past year thanks to NHS teams focusing on prevention. It is estimated that health checks have prevented approximately 380 major health emergencies in the region, including 180 heart attacks and 200 strokes. Health checks can show signs of diseases before symptoms appear, leading to more effective treatment and better outcomes.
Find out more about the preventative work that’s being done by our primary care teams by following the link in the comments.
Here’s a snap of a patient having a health check at St Andrew’s Medical Practice in Salford.
If you’ve been invited for a health check, make sure you attend, even if you feel fine – it could save your life!

Good News Alert!
Back in September, members of Eccles and Irlam Primary Care Network held a Charity Ball at the Lowry Theatre in Salford to rise money for the Mustard Tree, an Eccles-based charity that combats poverty and prevents homelessness.
The event was attended by staff from across the network which consists of seven practices: St Andrews Medical Practice, Monton Medical Centre, Springfield Medical Centre, Salford Primary Care Together, Chapel Group Medical Centre, Irlam Clinic and Mosslands Medical Practice.
They raised an incredible £1637 for the mustard tree and the network will continue to work closely with the charity, encouraging donations to their food club which supports local residents who are struggling with the cost of living. Photo shows Dr Tom Tasker - PCN Clinical Director, Georgina McNulty - PCN Managing Director and Candice Saunders - Executive Assistant presenting the money raised to the Mustard Tree.
Great work team!

Residents, communities, public service leaders and the Mayor of Greater Manchester share why it’s time to find new ways of supporting people and communities in everyday trusted places. Explore what Live Well is and why it matters – a journey from policy to practice, from neighbourhoods to boardrooms.
