Successful double microvascular lymph node transfer with double DIEP flap to simultaneously reconstruct both breasts after mastectomy for breast cancer, treat lymphoedema in the left upper extremity and prevent it on the right arm. Apart from technical expertise and experience required for one of the most complex reconstructive procedures, preoperative planning coupled with understanding of lymphoedema surgery and microsurgical breast reconstruction can result in great improvement in patients quality of life.

It was a great honor to be an invited faculty at World Lymphoedema Surgery Symposium in Barcelona, among top experts in the field. I presented my ‘Triple flap technique’ of simultaneous microvascular breast and lymphoedema reconstruction in skinny patients with lymphoedema (and inadequate volume in the abdomen) with most of my upper limb lymphoedema patients being garments-free long term. Special thanks to prof. Masia, dr Gemma Pons and all my friends from Barcelona who put an enormous effort to organise this amazing conference despite c19 restrictions. Hopefully we meet again soon 👏👏👏

Before and after LVA super-microsurgery (Lymphatico-Venous Anastomosis) for Breast cancer related upper extremity Lymphoedema. Before LVA patient could only wear a very large ring, but only 2.5 weeks following LVA super-microsurgery, there is significant reduction in her swelling, allowing her to wear her own normal size ring. Super-microsurgical bypass performed between 0.3mm lymphatic pathway and corresponding vein under 38x magnification (Zeiss Tivato microscope) and ICG technology (Fluobeam- Fluoptics). Patient continues to be completely off garments after surgery and very happy with early results.

Before and after LVA supermicrosurgery (Lymphatico-Venous Anastomosis) for cancer related right lower extremity Lymphoedema. Only 4 weeks following LVA super-microsurgery for right lower limb lymphoedema, there is significant reduction in her lower limbs swelling, allowing patient to remain off garments. Super-microsurgical bypass performed between 0.3mm lymphatic pathway and corresponding vein under 38x magnification (Zeiss Tivato microscope) and ICG technology (Fluobeam- Fluoptics). Patient continues to be completely off garments for right lower limb after surgery and very happy with early results. Targeted liposuction previously performed to left lower limb shows stable results as well. Combination of techniques and appropriate planning and understanding of each individual patients' problem are keys for success.

This will be a another great opportunity to collaborate between European microsurgeons and US colleagues to advance our specialty. I would like to invite our American colleagues to attend EFSM.
