Practice Nurse / Nursing Associate (if trained in respiratory)

Pay: Dependent on experience

Job Description:

The role will predominantly involve the provision of care of asthma / COPD patients, including assessment, screening and coordinating treatment plans and ensuring patients are reviewed at required intervals. In addition, the ideal candidate will be proficient in all aspects of Practice Nursing; including cervical screening, vaccinations (including travel vaccinations).

· Minimum 2 years’ experience in the managing respiratory (asthma and COPD)

· Experience of cervical screening

· Experience in vaccinations

· Minimum 2 years’ post registration experience

· Experience of working in accordance with the NMC code of professional conduct.

· Experience of SystmOne clinical system

The post holder is an experienced nurse who, acting within their professional boundaries, will provide high-quality, evidence - based respiratory care to patients within the GP practice, including the assessment, diagnosis, management and review of patients with chronic respiratory conditions such as asthma and Chronic Obstructive Pulmonary disease management, supporting long-term condition management, prevention education and health promotion.

Clinical Duties

Deliver specialist respiratory clinics for patients with:

Asthma
COPD
Perform comprehensive respiratory assessments including:
Spirometry testing and interpretation
Peak flow monitoring.
Inhaler technique assessment and optimisation.
Diagnose monitor, and manage respiratory in line with NICE and local ICB guidelines.
Develop and implement, and review personalised asthma ad COPD care plans
Manage acute exacerbations and provide urgent care assessments where appropriate.
Lead on our respiratory QOF indicators.
Deliver vaccination programmes (e.g. flu, pneumococcal, childhood immunisations and travel vaccinations.
To provide high quality cervical screening service in accordance with the NHS Screening Programme.

We are inviting carers to take part in a short survey to help shape a new information and support resource for:
• Carers who care for someone living with or beyond cancer
• Carers with cancer

The project is funded through the Cancer Related Health Inequalities Grant, coordinated by the Lincolnshire Voluntary Engagement Team (LVET).

We will create a practical resource to help carers access the information and support they need at the most appropriate time.
By completing the survey, carers can help us understand:
• What information and support carers currently receive
• What information has been most useful
• How information could be improved, including the content, format, timing, or how it is shared.

Responses will directly inform the development of the resource, ensuring it provides clear, practical and timely guidance for carers.

The survey also includes an option to be more involved in the project through case studies and feedback.

NHS Lincolnshire Integrated Care Board (ICB) has launched a new survey to hear from people who use Stoma, Bowel, Transanal Irrigation (TAI) or catheter products, along with their carers, about their experiences and what could be improved. We are asking for your support in promoting this important opportunity to the people and communities you work with.

The survey aims to understand what is working well, what could be improved, and how services can be shaped to better meet local needs. The feedback collected will support the ICB’s review of how Stoma, Bowel, TAI and Catheter care products and services are provided across Lincolnshire.
