Need help untangling a complex situation — fast?
When life becomes overwhelming, the most urgent issues are often the hardest to see. We help individuals and families make sense of what’s happening, identify what needs addressing first, and move things forward safely with the right services.
Examples of situations we regularly help with:
• An older person discharged from hospital where care, housing safety and services weren’t aligned
• Families navigating dementia or cognitive decline and unsure what support should be in place
• Someone struggling with incontinence or recurring UTIs who hasn’t been properly assessed
• A vulnerable adult living in unsafe housing needing urgent repairs without being taken advantage of
• Families living far away who need trusted local support to coordinate things properly
• Unpaid carers who are exhausted, overwhelmed, and unsure what help they’re entitled to
• People confused about Direct Payments vs Local Authority–managed care and the impact on existing support
What we do (non-clinical / non-regulated):
• Help you understand your options and next steps
• Support you to communicate with services (with consent)
• Prepare questions for GPs, hospitals, Adult Social Care, housing and benefits
• Coordinate practical issues like safety works, quotes, downsizing or moving
• Support unpaid carers with guidance, signposting and training options
• Support self-employed carers/PAs with checks, training options and secure record systems
• Safeguarding: if serious risk is identified, we may need to escalate appropriately
You get two highly experienced professionals working together — not just a one-hour conversation. Time is spent before and after sessions reviewing, planning and coordinating so actions are safe and proportionate.
Cost:
£150 – includes pre-session questionnaire review and first session booking.
Availability:
✔ Limited weekly slots to keep response times safe
✔ Priority slots held for high-risk situations (hospital discharge, safeguarding concerns)
