Meet Deepika Lawrence, our PCN Occupational Therapist. She led the Over 75s project with valuable support from the surgery’s Care Coordinators.
Questionnaires were distributed to all patients aged over 75 who were not currently on an annual recall pathway. Based on their responses, patients were followed up by our Occupational Therapist and either referred to appropriate services or offered a home visit for a comprehensive assessment.
Findings from the project showed that patients with identified needs experienced a significant improvement in their quality of life following the interventions provided.
This proactive initiative was funded through a BLMK Research and Innovation grant, in partnership with the University of Bedfordshire. The project was also presented as a poster at the recent EPIIC Conference, as illustrated in the image with Deepika.
As a PCN, we remain committed to enhancing patient care through innovative approaches. We are grateful for the engagement and participation of our patients, whose involvement makes projects like this possible.
