ANATOMY OF THE CERVICAL SPINE 🦴🦴

There are 7 vertebra in the neck, increasing in size the further down. As the neck is not as weight bearing as the lumbar or thoracic regions, the vertebra are much smaller in comparison
In between are the intervertebral discs to help with cushioning and force transmission

Holding all this together are the ligaments connecting the bones and the muscles to initiate movement and stabilise the head.

Nerves exit above each corresponding vertebra until the thoracic spine where they exit below, meaning there are 8 cervical nerve roots but only 7 vertebra

Assessment of the thoracic spine!

This is how I do a general assessment of the thoracic spine ⬇️

Assessments are a vital part of an appointment as it helps determine the diagnosis and treatment plan

ANATOMY TIME!
Thoracic spine

The thoracic spine is made up of 12 vertebra and like the rest of the spine has facet joints and intervertebral discs to help with support and movement.

Your RIBS also attach onto your t-spine and each vertebra has a rib attached either side giving you 24 ribs overall.

Due to the ribs, the t-spine has the least amount of movement in it with most moment seen in flexion and rotation.

The t-spine has a kyphotic curve meaning it curves outwards. This, along with the lordotic (inward) curve of the low back and neck, helps provide a spring like mechanism for the back to help with load distribution

As you can see from the image the vertebra get bigger the further down, this is because the further down the spine the more force is put through the spine - see the last picture for a fun fact!

MYTH BUSTERS!! 💥
This week it's all about busting myths surrounding the lumbar spine.
Here are some of the common myths about the lower back...

1. A disc 'slips' out

A common phrase I hear is 'slipped disc'. This is not what happens and creates a confusing image that the discs slip out of place. In fact, what people mean is a disc herniation or bulge where part of the disc is pushed out... not the whole thing!

2. A joint is popped out of place and a therapist pops it back in

Again, I hear a lot of people saying that their joints have popped out of place and they went to see a therapist who popped it back into place... if this was true you would probably be in hospital!
What more likely happens is that the joint can get irritated and inflamed and a therapist may use techniques to help improve motion in the joint.

3. Rest is best

Many people believe that when your back hurts you need to rest. This is in fact the worst thing you can do and the best thing to do is gentle exercise - walking is best.

4. Pain = damage

The most obvious thought when you have pain is that it is now damaged, however, this is not always true and perceived pain can be caused by other things such as degeneration around the discs or joints. OR perceived pain can be referred to the back from other places. This leads to thinking you have to rest to prevent further damage - see above for why rest isn't always best.
