Today we celebrate an incredible milestone — 15 years of Kelly Hendra being part of the Moderncare family.

We are looking for a kind, bubbly and reliable Community Carer to join a long-standing support package in Neath.

This is a wonderful opportunity to support a lady who has been with Moderncare for over 10 years. She enjoys getting out and about in the community, socialising and maintaining her independence — and she’s looking for someone positive and caring to support her.

You will be supporting one lady in her own home and out in the community.

The role includes:
• Supporting with community outings and social activities
• Encouraging independence
• Providing companionship
• Supporting with daily routines
• Providing personal care with dignity and respect

We’re looking for someone who is:
• Warm, bubbly and friendly
• Caring and patient
• Confident supporting in the community
• Comfortable providing personal care
• Reliable and committed

Experience is welcome but not essential — personality and attitude are key 💜

NEW VACANCY - CLIENT LIAISON OFFICER

We are currently looking to recruit a client liaison officer to join our growing office team.
Part time role – 3 days a week 9am-5pm (work pattern to be agreed– weekdays only)

The main duties of the role include:
• Initial assessments
• Setting up care packages
• Risk assessing the home environment
• Creating Care plans that are tailored to each individuals needs
• Completing reviews with Social workers and other health care professionals
• Reviewing the care plans on a three-month basis or more frequently when changes are required
• Office duties that include liaising with our clients, families, health professionals, and care team
• Making referrals to relevant bodies and health professionals

To apply for the role we do require:
• That’s you have excellent IT skills and are able to work with drop box, word, excel, share point and other IT systems
• Full UK driving licence – as you will be visiting individuals in their own homes
• Previous experience in a similar role
• Experience / good understanding with writing care plans
• Experience with assessing packages of care.

Rates of pay to be discussed and agreed depending on experience
