Care Coordinator – Urgently Required

Ultra Care Health Professionals are seeking a dedicated and experienced Care Coordinator to join our team urgently. This is an exciting opportunity for someone passionate about delivering high-quality care services while supporting carers, clients, and families.
Key Responsibilities:
• Effectively coordinate and manage care packages to ensure service users receive timely, person-centred care.
• Liaise with clients, families, social workers, and healthcare professionals to maintain high standards of care.
• Create and update care plans in line with service user needs.
• Organise and manage staff rotas, ensuring shifts are covered efficiently.
• Provide support, supervision, and guidance to care staff.
• Respond promptly to emergency cover needs and safeguarding concerns.
• Maintain accurate records in line with company policies and regulatory requirements.
Requirements:
• Previous experience as a Care Coordinator or in a similar role within domiciliary care is essential.
• Excellent communication, organisation, and leadership skills.
• Ability to work under pressure and problem-solve effectively.
• Good IT skills, including the use of care management systems.
• Knowledge of CQC/CIW regulations and compliance.
• Driving licence and access to own vehicle (preferred).
