It really is sad to read stories like this. We hear so many people say 'this doesn't apply to us' or 'it's health and safety gone mad' but the truth is it really can happen and sadly still does!
This could have been prevented had the company:
-Undertaken a suitable and sufficient assessment of the risks associated with workplace transport;
-Addressed the movement of HGVs in its site rules;
-Eliminated, as far as possible, the need for HGVs to reverse;
-Provided any aids or assistance to HGVs that had to be reversed;
-Displayed any signage to warn of the hazards arising from the movement of HGVs; and
-Controlled when HGVs would access the site.
