We are Precious Hope Care, a domiciliary care service in Northampton, who are passionate about supporting adults with care needs to stay in their own homes. We are looking to recruit a reliable Team Leader, to provide person centred care for our elderly/disabled clients, to enable them to remain living in the comfort of their own home. This rewarding position requires an enthusiastic and dedicated individual who puts others at the heart of all they do.
Main Duties Include:
• To carry out all client assessments for new clients including risk assessments as part of the Assessment process.
• To regularly monitor and review clients, updating documentation including support plans as required.
• To ensure all client visits are always covered.
• To be part of the on-call team in case of an emergency on a rota basis case of emergency.
• To liaise closely with Care Coordinators in relation to recruitment, allocation of Care Workers and the development of care & support plans.
• To allocate suitable alternative Care Workers when an existing Care Worker notifies that they are unable to attend a call.
• To be actively involved with the induction of all new Care Workers.
• To help identify any additional training needs of Care Workers.
• To report any changes in a Care Worker's or customer's schedule to the Care Coordinator.
• To monitor, supervise and mentor all Care Workers in the field.
• To provide support to all Care Workers in the field.
• To monitor all customers for service and Care Worker satisfaction.
• To ensure accuracy and completeness of all documentation held in customers' homes.
• To ensure that all written reports and administration is up to date
• To maintain close liaison with both the Registered Manager and Care Coordinator. Team Leaders are responsible for the efficient running of the service and should vary their hours of work in order to effectively monitor and support all Care Workers and customers.
Requirements needed to become Team Leader at Precious Hope Care:
• Position will be subject to satisfactory DBS and references.
• Full clean valid driver’s license and own transport is essential
• Precious Hope provide full training

Precious Hope Care provide care services to vulnerable adults in their own homes in Leicester and Northampton. We are now looking for a Care Coordinator to work in our busy office based in Leicester

This is a full-time post that requires someone with exceptional communication and organisational skills. Experience within the care industry is essential as well as knowledge of scheduling software such as Care Planner
We do require a candidate that has experience of co-ordinating homecare

Responsibilities:
• Ensure that the Quality of Service Provision meets with policies and expectations.
• Ensure that Regulatory Requirements are met.
• To implement a high-quality service through the effective matching of service user needs with suitably qualified and experienced care workers.
• To facilitate effective communication between the Organisation, service users, care workers and outside Health Professionals and Local Authorities.
• To ensure a high level of records are imported and held on Care Planner for both Care Workers and Service Users.
• To manage and monitor the branches rostering requirements of its care team
• To ensure all aspects of health and safety requirements are adhered to as specified within the Health and Safety Policy and procedure and in accordance with training given.
• At all times observe and respect the service users’ dignity, privacy and independence as far as is practical.
• To take the On-Call responsibility
• You must be a team player and be able to trouble shoot when the pressure is on. Ideally you will already be in a similar position.
