Welcome to your new role Leanne!

The Service Leader role has been created as part of our management restructure, some of Leanne's responsibilities include:

· Improvement of people’s quality of sleep (QoS) & quality of life (QoL) through use of the PERMAH model

· Implementation and supervision of practice leadership model

· Ensuring people are supported to develop and maintain their independence

· Ensuring people are appropriately safeguarded from abuse and that action is taken in the event of potential / actual abuse

· Ensuring people are supported to ensure they have access to health services when these are required, including annual health checks and medication review

· Overseeing health and safety systems are implemented and monitored to minimise risk of harm to the people we support and employees

· Ensuring quality assurance systems are utilised effectively and reviewed at regular intervals

If you need support with anything connected to the service being delivered to the people we support, Leanne is the next point of contact (after Practice Leaders).

Please note, this is part of a larger restructure within the management team (which means the Care Coordinator role will no longer exist).

For queries directly relating to the care of the person (or people) you support, your Practice Leader will be the first point of contact.
