New draft guidelines - Pelvic floor dysfunction: Prevention and Non-Surgical Management - June 2021.

The Chartered Society of Physiotherapy's (CSP's) Pelvic Obstetrics and Gynaecological Physiotherapy (POGP) have welcomed the draft NICE guidelines recommending supervised pelvic floor training programmes for women with pelvic floor dysfunction.

POGP chair Dr. Kate Lough added " This guideline is a very welcome step towards a strategy for lifelong pelvic floor care for all women, helping to reduce the normalisation of pelvic floor dysfunction at key life stages such as childbearing and older age. "

The guidelines recommend a supervised pelvic floor muscle training programme of up to 4 months for women with pelvic organ prolapse and urinary/faecal incontinence (leaking).

In addition, to include pelvic health classes in schools and recommending a community based multidisciplinary team approach for the management of pelvic floor dysfunction.

Postnatal : Return to running/high impact exercise:

Lots of women ask me when they are ready to return to running after having a baby.
There has been a lot of research done in this area and safe to say there is not one guideline that fits all. The general advice suggests after 3 months postnatal but that depends on many factors.

Running is a high impact sport placing a lot of demand on the body. It has a 4.59 fold increased risk of pelvic floor dysfunction compared to low impact exercises. It is a fantastic form of exercise but just need to be fully prepared.

Postnatal women need time to heal especially the abdominal and pelvic floor muscles. Following vaginal delivery and c-section it can take up to a year to recover. Recovery of muscles, nerves and connective tissues can take between 4-6 months after delivery minimum.

There are some keys signs/symptoms that should delay your return to running such as a heavy or dragging sensation around the perineal area, incontinence or moderate to severe pain.

There are other factors that also need to be considered such as weight, pre-pregnancy fitness, breathing, psychological status, abdominal separation, sleep, breast feeding and much more......

Should you have any of the above symptoms make sure you have an assessment to check your pelvic floor muscles, and general muscle strength prior to commence running.

When you are ready to return to running, you should start 1-2 minutes at a very gentle pace. Maybe consider couch to 5k which alternates running / walking. Keep an eye on the key signs mentioned above and modify your programme accordingly. A total weekly running distance /time should not be increased by more than around 10% per week. Any pain should be kept mild (less than 3 out of 10 on a pain scale).

At NP Physio, I can offer a full assessment of pelvic floor muscles, load and impact assessment and strength testing to ensure key muscle groups are prepared to return to running/impact exercise.

You can then commence a full rehab programme at Fitwell studio using reformer / cadilac machines, which offers the perfect assistance / resistance for postnatal clients.

Any queries do not hesitate to contact me on info@npphysio.co.uk. There is plenty to do to ensure you are ready to return to running safely. πŸ‘

For the runners out there..... this is a programme to assist return to running following injury. It is essential to make sure you have a good technique throughout! If you can achieve this programme pain-free then you are good to commence running!

I just wanted to reassure all that physiotherapy services remain open across the UK no matter what level of lockdown restrictions are in place. We continue to follow the Chartered Society of Physiotherapy and government guidelines at all times.

The new National Institute for Health and Care of Excellence (NICE) guidance now recommends the non-pharmacological management of chronic pain, including the use of supervised exercise programmes (1:1 or classes), psychological therapies (mindfulness and/or cognitive behaviour therapy) and acupuncture. It states the importance of a holistic approach..

All of this can be offered at The Green Space. πŸ‘

I am still doing a brief 5 minute telephone consultation prior to face to face appointments to screen for covid symptoms and clinically reason all risks.

There will be hand sanitizer upon arrival at the door plus please bring along a face mask for yourselves.

Please visit my website (www.npphysio.co.uk) for further information.

Hello all, I hope you are well..

Good news....... the clinic will be opening in 2 weeks time!

I just wanted to mention that I will be following the government guidelines and also the Chartered Society of Physiotherapy and Physio First guidelines in relation to the covid-19 Pandemic:

- A screening consent form will be carried out over the phone prior to face to face appointments. This will include screening for covid symptoms, clinical reasoning for face to face, risk assessment and consent to track and trace. Upon arrival the client will be asked to sign this consent form and also at the beginning of subsequent sessions.

-Hand sanitiser and a face mask will be offered to the client upon arrival (a Β£2 charge will be required for the client's face mask).

- I will be wearing a face mask throughout and additional gloves and aprons with any contact.

-All appointments will be staggered and I will make a call for you to enter at the appropriate time to avoid use of the waiting area. The bathroom will be closed for now.

- The room will be thoroughly cleaned in between patients including any equipment used, door handles and plinth.

- Access to the clinic will be discussed over the telephone should a face to face be appropriate.

I just want to reassure you that I will do everything possible to keep any risks to an absolute minimum.

I look forward to seeing you all soon..

I attended a fantastic online course yesterday with Dr. Brinda Christopher covering tendinopathy injuries.

65-80% of our tendons are made up of type 1 collagen but following injury (one off or repetitive) we develop type 3 collagen causing the tendon to become weaker and less efficient. Another factor that causes pain following injury is neovascularisation (an increase in nociceptive pain nerve fibres).

Tendon injuries can affect many areas such as inner and outer elbow (also known as medial and lateral epicondylitis - tennis and golfers elbow), supraspinatus (rotator cuff shoulder), Achilles tendon, patella tendon (knee) and various other areas of the body.

There is evidence to suggest prescriptive exercise and other modalities can have a beneficial effect at stimulating type 1 collagen fibres and reducing the neovascularisation. As well as modifying your activities.

There are many stages of tendon injury....from a one off acute overload to a continued overload and long term degeneration. It's important to work out which stage you're at to follow a programme that is tailored to you.

There are many causes (too many to list) but most will be discussed during a 1:1 session (online during this current situation).

I hope you are all enjoying the beautiful sunshine 🌞.
