Here today with a little public service announcement: If your child has a speech and language delay, please have their hearing checked!
Of course, not every child with speech sound difficulties has a hearing impairment! However, sometimes, it can be a huge piece of the puzzle. Let me tell you about H...
H is now 4 years old and has been on my caseload for 15 months. He had a history of glue ear (a build-up of fluid in the middle ear, which can cause hearing loss) and had previously had grommets fitted. It was thought that this had resolved the issue.
When I met him at 3 years old, he had only 1-2 words and was showing outward signs of frustration at not being able to communicate. As words began to emerge they were extremely unclear. He was displaying many of the indicators of Childhood Apraxia of Speech - a very limited repertoire of sounds, vowel distortions, use of non-English sounds, inconsistencies and limited prosody. We worked with CAS approaches and he made progress, but it was slow. Three months ago, a follow-up hearing test revealed that his severe glue ear was back. He underwent a second procedure to fit grommets again.
The progress he has made in the last 3 months since the grommets were fitted has blown me away. His speech has gone from almost unintelligible to verging on age-appropriate. Once he was able to hear more clearly, the effectiveness of therapy skyrocketed. Suddenly he could hear me clearly! The biggest impact for his family has been his mood and behaviour - now that he can explain what he wants and how he feels, he is like a different child.
So when it comes to hearing - if in doubt, rule it out!
