Is it time for your annual health review? 🩺✨

If you live with a long-term condition, your annual review is a vital check-in to ensure you are staying well and receiving the best possible care. It is a great opportunity to discuss your health, review your medications, and ask any questions you might have about your wellbeing.

If you have not yet booked your long-term condition review for this year, please contact our reception team to arrange your appointment.

For those patients who are normally asked to provide blood pressure readings as part of their review, please remember to:
πŸ“‰ Complete 5 days of readings
πŸŒ… Take 2 readings in the morning and 2 in the evening (2x AM, 2x PM)
πŸ“ Bring these completed readings with you to your appointment

Having these readings ready helps our clinical team provide you with the most accurate assessment during your visit. We look forward to seeing you soon! πŸ‘‹πŸ₯

How to help someone sleeping rough in our local community 🀝🏘️✨

Seeing someone sleeping rough in our towns or villages can be a difficult experience, and you may be asking how you can offer support in a way that is truly helpful. The Orchard Partnership is pleased to share some practical and compassionate advice from our local outreach partners at Julian House. πŸ•ŠοΈπŸŒΏ

Here are some ways you can make a difference for someone in our community:

Acknowledge and connect πŸ—£οΈπŸ‘‹ Homelessness can be incredibly isolating. Simply making eye contact, offering a smile, or starting a brief, respectful conversation can significantly improve someone's wellbeing and sense of self-worth.

Offer practical help (but ask first!) β˜•πŸ§£ If you would like to give something, it is always best to ask the person if they are happy to accept it first. You might offer a warm drink, some food, or a blanket. Another helpful option is to "pay it forward" at a nearby cafΓ© so the person can collect a meal later.

Use StreetLink for non-emergencies πŸ“±πŸ“ If you are concerned about someone’s health or situation but there is no immediate danger, the best thing you can do is alert local services via StreetLink. This allows a professional outreach team from Julian House to find the person and connect them with the support and accommodation they need. πŸ‘‰ Notify them here: www.thestreetlink.org.uk

In an emergency πŸš‘πŸ†˜ If you see someone in urgent need of medical attention, or if you believe the person is under the age of 18, please call 999 or 111 immediately.

For more detailed guidance and to learn more about the services provided in our area, please visit the Julian House website: πŸ’» https://www.julianhouse.org.uk/post/what-to-do-if-you-see-someone-sleeping-rough

Thank you for looking out for the most vulnerable members of our community. πŸŒ³πŸ’š
