🦵IT BAND SYNDROME, KNEE PAIN AND TENSOR FASCIA LATAE🦵

The iliotibial band is a thick strip of connective tissue which runs down the outer thigh from the pelvis to the tibia, crossing both the hip and knee joints.

Many people struggle with IT band syndrome, especially common with runners. The main symptom is lateral knee pain, which is often mistaken for a knee injury. A tight IT band can cause friction and inflammation where it crosses the knee joint.

IT band syndrome is generally classed as an overuse condition with many contributing causes. It can be caused by restrictions or weakness in the muscles of the hip, namely tensor fascia latae muscle (TFL) and glute max, which insert onto the top of the IT band.

Many people advocate foam rolling for the IT band. Foam rolling is a useful practice for relieving muscular tension... but the IT band is connective tissue, not muscle! Likelihood is - by rolling the IT band itself all you’ll do is inflict pain! However, you can still use a foam roller to help relieve IT band pain - target the lateral hip muscles instead (TFL and glutes).

💥 TRIGGER POINT THERAPY 💥

Trigger point therapy is a neuromuscular technique which can be used to address areas of pain, tension and stiffness.

This technique involves direct pressure on a specific area of tension within the muscle. Pressure may be applied for up to 90 seconds or until the pain eases.

People often experience referral pain or sensations in other areas, this may highlight other areas of tension to be addressed.

This technique is very effective at treating and relieving pain. Trigger point technique is the basis behind self-release tennis ball massage 🎾

🌟 POSTURE CORRECTION AND MYOFASCIAL RELEASE 🌟

⭐️ What is fascia? Fascia is a vast network of connective tissue which covers and connects all structures of the body through a series of associated chains. Fascia is often responsible for issues in one area of the body being reflected in another area when one or more of these chains becomes restricted.

⭐️ Myofascial release targets the fascia which surrounds the muscles. It is particularly useful for improving posture and restoring movement by lengthening fascia which has become contracted and restricted.

⭐️ In a previous post we discussed anterior pelvic tilt, this is one of many postural deviations which can be addressed using myofascial release techniques. Another postural example is rounded shoulders, myofascial release can help to open up the chest and therefore allow the shoulders to return to a more neutral and functional position.

Guess which side has been treated first...?! 👀

👉Upper trapezius and levator scapula muscles are responsible for elevation of the scapula, or shrugging / lifting of the shoulders towards the ears.

👉Elevated shoulders is a common postural deviation and an area where many people carry tension and stress.

👉In this photo I had worked the right side first using a combination of soft tissue release, trigger point therapy and muscle energy techniques to release and lengthen the muscles mentioned. You can see the difference compared to the left shoulder which had not yet been treated.

🌟ANTERIOR PELVIC TILT, TIGHT HAMSTRINGS and LOWER BACK PAIN🌟

➡️Due to the relationship between the anterior and posterior muscle chains of the body, when hip flexors become shortened (often due to prolonged periods seated) the hip extensors of the posterior chain are forced in to a weak, lengthened state - this is the theory of reciprocal inhibition (will cover that in more detail in a future post). This can lead to an anterior pelvic tilt - pelvis tipped forwards and over exaggerated lumbar curve.

➡️This can also cause lower back pain - the spinal erector muscles are shortened as the pelvis tips forward, vice versa the anterior abdominals are weak and lengthened. Lower back pain may also be a result of compensation due to inactivity of the glutes.

➡️So that’s why doggedly stretching your hamstrings is largely ineffective, as they are already lengthened - this is what creates the feeling of ‘tightness’, rather than being short they are under excessive tension.

➡️The solution? Restore a neutral position of the pelvis so the muscle chains are able to work in harmony. A variety of therapy techniques can help to initially regain this balance, which then gives a solid foundation on which you can strengthen the stabilising and supporting musculature of the pelvis.
