Job Vacancy - Care Coordinator
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a crucial role in ensuring that our clients receive the highest level of care and support. You will be responsible for coordinating and managing the rostering of our clients, working closely with the registered manager, the healthcare professionals, care workers, and clients themselves.
You must have a full UK driver licence and your own vehicle as it is essential for the role.
Responsibilities:
- On-Call on a rota basis.
- Managing weekly rota's to ensure care calls are organised and well planned by using rostering software.
- Answering and actioning phone calls.
- To organise and coordinate care delivery.
- To drive the business forward.
- Liaising with the recruitment officer to fulfil rostering needs.
- Monitoring and actioning emails.
- Handing over information with relevant colleagues and healthcare professionals.
- Act as a liaison between clients, care workers, and healthcare providers to ensure effective communication and collaboration.
- Provide guidance and support to care workers, offering training and resources to ensure they provide the best care possible.
- Monitoring business performance and reporting to management.
- Confirming visits for finance.
- Respond promptly to client and care worker inquiries and concerns.
- Ensure compliance with all relevant healthcare regulations and standards.
- To participate in the care delivery in the absence of care staff. (This is paid for on top of salary)
Qualifications:
- Minimum Level 3 Health and Social Care
- Previous experience in care coordination or a similar role in healthcare.
- Strong communication and interpersonal skills.
- Excellent organisational and problem-solving abilities.
- Compassionate and empathetic attitude toward clients and their families.
- Knowledge of healthcare regulations and standards.
- Proficiency in using healthcare management software and tools.
