care. Sarah Lee’s mobile service means treatment is affordable and accessible to all. Most suitable for older adults and persons with well managed diabetes needing help with their footcare routine. Available in Stafford (surrounding areas subject to availability)

• Nail trimming
• Dry / Cracked heels
• Corns
• Callus and hard skin as required

Excluded - infections ulceration and chronic in-growing nails

Our Story

Difficulty getting to a salon or clinic ? Affordable mobile service for nail trimming and general foot health and care. Painful and unsightly foot and nail conditions can cause embarrassment, impact heavily on enjoyment of life, impair general health, and the ability to manage daily living activities. Sarah Lee’s home visiting service means treatment is affordable and accessible to all, providing relief from discomfort caused by conditions of the feet and nails Based in Stafford but available to some surrounding areas Treatments include • Nail trimming and correction • Dry / Cracked heels • Fungal nails • Corns • Callus and hard skin • Verrucae advice • Diabetic foot assessment and monitoring Fees £25 per visit, discounts for additional clients at same address No charge for cancellations or missed appointments - however repeated incidents will result in service being withdrawn

All clients must complete a registration and consent form before treatment

You may be aware that there are new regulations came into force in May 2018 regarding Data Protection. This means that Sarah Lee, has a duty to inform you of any personal information that we hold about you, why this information is taken, who has access to the information and how it is stored and used securely. This document has been compiled to explain this.

What Data will be collected/processed?

Your name, address, telephone number, email address, next of kin details, date of birth, GP details, medical history/details (including current medication)

For what purpose is the Data collected and used?

To enable Sarah Lee to be able to appropriately provide treatment to you. If appropriate/necessary to contact your GP with regard to any findings that could need onward referral if outside of our remit of treatment. We may also use your contact details to provide you with appointment reminders as necessary via telephone, SMS or email.

Who will have access to the Data?

Sarah Lee solely has access to all data held

AT NO POINT WILL ANY OF YOUR INFORMATION BE PASSED TO A THIRD PARTY

How will the Data be stored?

All information given is held in paper form and kept in locked storage. [Patient telephone numbers may also be stored on Sarah Lee’ mobile phone which is PIN protected)

How long will the Data be stored?

In line with the legal requirements of holding medical data, your record is stored for 7 years (or in the case of a minor until that minor reaches the age of 25 years) from the date of last treatment by us.

Please note that your right to be forgotten cannot override the legal requirement to keep medical notes for the mandatory period

When will the Data be destroyed?

All data will be destroyed after 7 years, or in the case of a minor once that minor reaches the age of 25 years
