Prime Support Service provides home and community support services for adults (18 years +) who have learning or physical disabilities, older adults and adults who experience mental ill health. We enable people to remain in their own homes or within supported living environments hence giving them independence, privacy, choice, and control over their lives. We are registered with Care Quality Commission and have demonstrated during the registration process the high standard of the work we provide. We will provide a seamless service that is person centred and tailored to meet individual needs of all those we work with. We will deliver reliable services and are keen to empower service users to maximise their independence. We will provide support services concerning all daily living skills, personal needs and desired social and leisure activities to suit each individual. We will support and empower Service Users to design their own person centred care plans and we strive to deliver support services tailored to meet each individual need. We provide support within the home environment and out in the community, maximising social and leisure activities to the fullest extent. We are more than just support. Our focus is on creating a client centred service, whereby you choose every aspect. Each and every person is unique and different, and it is our aim to accommodate these individual characteristics in an environment that promotes independence and improves quality of life.
