Heartwood Medical Practice We are a GP surgery based in Swadlincote, based at Swadlincote Health Centre on Civic Way, DE11 0AE.

Out team includes GPs, Advanced Nurse Practitioners, Practices Nurses, HCAs, Health Coach and a Social Prescriber, all supported by our admin team. We are a team of 6 GP's, 2 Advanced Nurse Practitioners, 4 Practice Nurses, a Practice Pharmacist, a Health and Well-being Worker, Social Prescriber and 2 Health Care Assistants, as well as a reception and admin team. Our aim is to provide an excellent medical service to our patients

💊🎄Medicines Order Line - Christmas Ordering

As Christmas, New Year and multiple bank holidays are approaching, we expect Medicines Order Line to experience high demand and higher than usual call volume.

To help with the increased pressure, if your medication is due over the Christmas period (w/c 22nd and 29th December), the MOL will accept your medication request 14 days prior to the due date.

Controlled drugs will be dealt with in the normal way, whereby future dating them to the date due on the clinical system. These will not be issued any earlier. If the due date falls on a weekend of bank holiday, then they will be future dates to 2 working days prior to this.

The MOL will be close on the 25th December, 26th December and 1st of January

😷 We politely ask that from today, if you are attending the surgery for an appointment and have a respiratory illness, that you wear a face mask.

Due to a rise in respiratory illnesses and winter pressures, we need to keep our colleagues and patients safe.

Masks can be found on the front desk in our reception area.

↩️ The patient entrance to the Adult Emergency Department (A&E) at Royal Derby Hospital is moving this weekend, alongside other urgent and emergency care services at the hospital.

🏥 This will allow us to expand the space we have available to care for our emergency patients throughout winter. If you are coming to Royal Derby Hospital for any of the following departments, we have shared helpful information on our website on how to find them:

• Adult Emergency Department (A&E)
• Medical Same Day Emergency Care (mSDEC)
• Urgent Treatment Centre (also known as Blue Urgent Care)
• Discharge Assessment Unit (DAU)

🔍 All departments will be clearly signposted, but if you need support access the new locations, please talk to a member of staff.

‼️‼️Please Note Heartwood Medical Practice is not part of the Out Of Hours Walk In Service, this service starts at 6:00pm on the 2nd floor.

Patients are unable to use our waiting room whilst waiting for the service to open, please use the side door to gain access.

Swadlincote bus station car park will be closed from today until Sunday, for the Christmas lights switch on, so please leave plenty of time to find a parking space before your appointment, incase our car park is full.
