This check is different from other health checks or appointments you may already have.

Your GP should invite you. If they haven’t, you can contact your GP and ask for the check.

It’s your chance to talk about your health and how you’re feeling, ask questions, and get support.

Age UK are currently undertaking a county-wide public survey and would greatly appreciate your support.
