Introducing our Care Coordinator – Sarah
We’re pleased to let our patients know that we now have a Care Coordinator at the practice.
A Care Coordinator helps patients get the right support at the right time.
They can:

• Help you navigate health and care services
• Support with organising appointments and follow-ups
• Link you with community and support services
• Assist patients with long-term conditions or more complex needs

They are not a clinician, but they work closely with our GP and nursing team to make sure your care is well organised and that you know what support is available to you.
If you feel you might benefit from support from our Care Coordinator, please contact reception and we can arrange for Sarah to get in touch.
We’re here to help make your care as smooth and supported as possible.
