I’m not the biggest fan of foam rollers!

But they aren’t entirely useless, just used in the wrong way and misinformation is everywhere

WHAT IS IT?
It’s a technique called self-myofascial release (the release part of that terminology really annoys us)

WHAT DOES IT DO?
You can increase your stretch tolerance in the short term by reducing your pain perception
Increased blood flow** but definitely not as much as riding a bike for 5mins

REHAB FOR TENDINOPATHY!

Recent consensus statement for UK military on the management of Pateller and Achilles tendinopathy💪

Don’t look to much in to it, the military make a great population for studies like this as they follow gold standard practice & have the ability to reassess & make a study out of their findings!

As it has been known for many years now, the best rehabilitation practice relies on the use of exercise based management. With slight differences between the patellar and achilles👇

Patellar
-isometric and isotonic exercises to start with
-heavy slow resistance and tendon loading progression
-HSR & PTLE are preferred to eccentrics

Achilles
-isometrics to start with
-either HSR or eccentrics to progress
-lower volume protocols equally as effective as high volume protocols

Manual therapy
-insufficient evidence for most modalities including trigger point, massage, stretching
-acupuncture, ultrasound, ice or laser pretty much useless
-some short term pain relief can be gained from friction and taping

In short… if your sports therapist or physiotherapist doesn’t have access to weights on sight. You’re in the wrong place!!

Hopefully, with the increase of evidence based practitioners these days, they will at the very least advise you on these protocols. But how can they effectively test you and find the right dosage for you🤦🏻‍♂️

WHY YOU’RE ACHILLES IS HURTING?

The Achilles tendon;
-largest and strongest tendon in the body
-connects calf muscles to your heel
-stores and releases elastic energy during walking/running
-takes very high loads (6-10x body weight when running)

With it being a load bearing energy storage tendon, it’s prone to overuse injury!

RISK FACTORS
-age
-reduced calf strength
-limited ankle mobility
-diabetes, hypertension, obesity etc
-sudden increase in training load
-introduced more hill running
-poor footwear
-previous limb injury

To name a few!!

TYPES OF ACHILLES TENDINOPATHY
-mid portion, 2-6cm above the heel & generally caused by overuse
-insertional, at the insertion point on the heel & generally caused by compression + bone spur

TENDINITIS OR TENDINOPATHY?🦵

Achilles pain is a common injury we come across at this time of the year!

New training plan with a sudden increase in training load being one of many influencing factors📈

But what’s the difference between tendinitis & tendinopathy?

In the simplest form;
-TENDINITIS; Acute term and refers to inflammation at the injury site
-TENDINOPATHY; Chronic term and refers to degenerative stage of the injury site

Key symptoms;
-pain at the tendon
-morning stiffness
-pain during loading
-reduced calf strength

We could describe for hours all the information referring to Achilles tendinitis/tendinopathy alone, so we’re breaking it down in to seperate posts!

Once upon a time, it was thought to be a pathology…

These days, barefoot gym shoes, people squatting bare foot & we are encouraged to spend more time out of trainers than in them!

But does having an anatomical ‘flat foot’ increase your injury risk?

The research says no. Normal feet and normal arches appear in a variety of ways!

It seems to be only the extreme cases of either end of the spectrum that *may* increase injury risk.

A more recent BJSM paper stated “…flat feet should be considered as healthy anatomical variants. It is imperative to abandon the outdated notion, common in both research and clinical settings, that having flat feet is problematic and makes individuals at high risk of sustaining musculoskeletal injuries.”

Do you have an opposing view? Been managing these conditions for years due to popular belief?

Because I know I have! This game is constantly evolving & changing and it’s our job as practitioners to stay on top of the research!
