The Community Link Worker is a member of your GP surgery team, offering one-to-one appointments for people age 18 plus.

Community Link Worker provide advice & support on a wide range of social issues affecting Health & Wellbeing E.g benefits & welfare, housing, employment & access to education.

At the moment we are offering digital services with a view to face to face appointments if required.

Referrals can be made on request, by contacting your GP surgery. You can also self-refer by giving us a call on 01942 836967 or text CLW to 61825 and we will give you call back.
