🧠🔍 Today on Diagnostic Diaries!
We saw a sweet 5-year-old Springer Spaniel who presented with a severe head tilt, jaw clenching, nystagmus, and was non-ambulatory on arrival.
An MRI of the brain revealed several significant findings:
🧩 Left-sided cerebello–medullary pontine angle intraparenchymal ring-enhancing lesion
• Most consistent with abscess vs necrotic neoplasia
🧠 Suspected left-sided trigeminal nerve enlargement
• Differentials include lymphoma, immune-mediated or infectious neuritis, and less likely nerve sheath tumour
🔎 Left-sided medial retropharyngeal lymph node enlargement
💪 Left-sided temporalis muscle atrophy
• Likely secondary to trigeminal nerve involvement
💢 Left-sided pterygoid myositis
👂 Left-sided otitis media with effusion
• Also likely secondary to trigeminal pathology
These findings give us a clearer picture of this patient’s complex neurological presentation and help guide our next steps in care. Sending this lovely spaniel all our hope for improved comfort and recovery. 💙🐾
