What is EAR WAX? 🤔

Everybody produces ear wax (cerumen). A small amount of ear wax is produced by the body to help fight off infection, it keeps dust, dirt and debris from getting deep inside of the ear, and also moisturises the ear canal. Some people may produce little wax and this may cause itchiness in the ears, and some people produce too much ear wax.

Once ear wax has served its purpose it eventually migrates to the surface, dries up and falls out of the ears without us even knowing. Some people for reasons unknown produce more wax than usual. Excessive ear wax can accumulate in the ear canal and cause symptoms such as:

👂 Ear pain
👂 Ear fullness or a plugged sensation
👂 Hearing loss
👂 Tinnitus (ringing in the ears)
👂 Ear odor
👂 Popping sensations in the ear throughout the day
👂 A feeling of water being blocked inside the ear after swimming or a bath
👂 Ear infection

Many people may try to unblock their ear using cotton buds in an attempt to clean the ear. This practice is dangerous for impacted ear canals and can block your ear further by pushing the wax even deeper down the ear canal. This will exacerbate the symptoms above and may even lead to eardum rupture. People most at risk of impaction are hearing aid users and people that use cotton buds or people that use ear plugs to sleep as the wax has limited time to naturally clean out at the surface.

The NHS guidelines state that if you suspect earwax build up you should speak to your pharmacist who will recommend drops to dissolve the wax. The earwax may fall out on its own within a few days. If the ear hasn’t cleared in 5 days then that would be an appropriate time for microsuctioning.

What is EAR MICROSUCTIONING? 👂🤔

Microsuctioning is an ear wax removal technique which is safe and comfortable. It involves the use of a microscope or surgical loupes with a bright light attached which provides the technician a clear and magnified view of the ear canal. A low pressure suction device is then used to gently suction any ear wax obstructions. This technique may be coupled with a gentle water rinse to remove deep impacted wax or any loose fragments that remain in the canal after microsuctioning.

NICE guidleines state clearly that manual syringing that was common practice until recently should never be offered to patients, the practice is not safe and can cause perforation of the eardrum. Microsuctioning should be offered as an alternative. Most GP surgeries at present are not offering the services.

The mobile clinic is brought to your home and set up takes about 10 minutes, I will go through a risk assessment and ear history and finally a consent form which includes the risks of the procedure. I will examine your ears and proceed with microsuction if necessary. The procedure may be simple and take 10 to 15 minutes and some may take a little longer depending on the nature of the impaction or depending also on the patient. Finally a written report and summary is given for your perusal.

Microsuctioning is safe for 6 month olds and above, it is important that the patient is able to sit still for a period of time during the procedure. To prevent wax build up it is suggested that the procedure is repeated every 6 to 12 months.

Jennifer Oakley has a nursing background in excess of 20 years, with vast experience working both in the community and in the acute settings. Jennifer has had specialist accredited training in aural microsuctioning through the auspicious Ear Care Academy based in the Charing Cross Hospital 🏥
