🌟 What is a Primary Care Network (PCN)? 🌟

At Featherstone Family Health Centre, we’re always working hard to improve the care and support we provide for our patients. You might have heard the term “Primary Care Network” (PCN) before, but what exactly does it mean?

Seisdon Primary Care Network is a group of GP practices across the area, that work closely together with other healthcare professionals to provide a more comprehensive range of services. By teaming up with other local practices, as well as specialists and community services, we’re able to offer better and more personalised care for our patients.

👩‍⚕️ What does this mean for you?

✅Better access to a wider range of services, including physiotherapists, dietitians, health and wellbeing coaches, paramedics, general practice assistants, mental health practitioners, pharmacists, social prescribers, and care coordinators!

✅Improved care coordination ensuring you get the right support at the right time.

✅More specialists available to help with specific health needs in your community.

✅Stronger connections between healthcare professionals working together for your well-being.

✅People living with or affected by cancer.
✅Volunteers giving their time to directly support people living with cancer.

The first sign of a stroke might not seem like much.

Like not being able to raise your arm, or struggling to smile, or slurring when you speak. Even if it doesn’t seem like it, any sign of a stroke is always an emergency.
