There is hope for Diabetes--the Silent Killer.

Medical Nuggets for this week: Diabetes , the silent pandemic - get protected._

Diabetes is a challenging problem for public health worldwide.

Sugar, glucose, is an essential energy source of our body. The hormone insulin is required for cells to absorb glucose and convert it to energy.

Diabetes develops when the body can't produce or use insulin effectively, leading to high glucose levels in blood stream.

Two main types exist:
Type 1, once known as juvenile diabetes, characterized by destruction of the insulin producing cells in the pancreas, usually accounts for about 10 - 15 % of cases.

Type 2, the most common form of diabetes , accounts for about 85 - 90%, is characterized by reduced production of insulin and inability of the body to respond fully to insulin. So diabetes is not necessarily "Sugar illness", that is to say caused by eating too much sugar per se.

Diabetes is a major cause of heart attack , blindness , stroke , kidney disease and lower limb amputation.

People with diabetes often have no symptoms at first. A person can have diabetes for years without knowing it - it is sneaky and hence a Silent Killer

The good news is, if you discover high blood sugar (diabetes) early , lifestyle modifications alone can prevent or delay the disease and in some cases return their blood glucose to normal.

Normal fasting blood sugar should be less than 6mmol . Anything more than that, seek care!

THE BREAST; An ominous threat to life. (Part 2)

BREAST cancer is the commonest cause of cancer death in Ghana, and most developing countries.
Even though the incidence is far lower that of Europe, our mortalities ( the numbers that die from it) far exceeds that of Europe.

It is believed that 1 in every 8 women will get breast cancer in her life time.

Whereas its common in elderly women, from 60 years onwards, in Africa, and for that matter Ghana, its common in younger women, between 40-50 year A lot more younger women are getting breast cancer than in Europe.

The youngest I have seen in Ghana is 25. Have managed a lot of young ladies in their late 20s and early 30s with breast cancer.

It is believed that annually we diagnose about 2 500 cases.....that is new cases.

Our mortalities are high because majority of our patients report with advanced stage of the disease, that is they report late, hence miss the chance of cure.

Men also can get breast cancer. In Ghana about 2-4% of breast cancers occur in men.
I have seen 2 cases in the last 4 years.

Breast cancer , if not detected early, is indeed a major threat to life.

There are other benign breast conditions, which in itself does not threaten ones life. But because of fear of cancer, a lot ladies suffer significant psychological effects even with benign breast diseases that impacts on their quality of life.

In our subsequent discussions, we will look at some of these benign breast diseases and later conclude on breast cancer.

It is important to emphasize that early detection of breast cancer guarantees cure.

One of our key objectives this month is to arm ourselves with skills/knowledge that will help us to detect breast cancer early.

THE BREAST; An ominous threat to life ( Part 1)

The Breast is a modified sweat gland. The Male breasts are an undeveloped version of those on the female while In females, underdeveloped breasts or partial or complete removal, which may be necessary to treat certain diseases like breast cancer, may cause significant emotional distress.

The bulk of the breast tissue is adipose tissue with interspersed connective tissue. The glandular tissue, that is part responsible in making breast milk is about 15 - 20 lobes in all females, irrespective of size of breast.

Hence the difference in sizes of breast is mainly due to adipose tissue, that is fats. Glandular tissue is same for everyone irrespective of size.......and hence size is not a marker of fertility.

One of the main function of the breast is the production of milk to nourish the infant. Breast milk offers significant immunological protection, and prevents certain allergies and autoimmune diseases to a large extent.
Hence the advocacy for exclusive breastfeeding at least for the first 4-6 months of the newborn. Its the best care one can ever offer to your child, as its components second to none; NO feeding formula in the whole world had its immune components.

This week, 1st to 7th August, is recognized as World Breastfeeding Week. We therefore acknowledge all our mothers/wives who breastfed us and our children for that labor of love.
We encourage all would be mothers to continue the tradition.

The breast, apart from milk production, also has a psychological function in that it is associated with sexuality. Research have demonstrated that there are three areas in the brain that lights up when the ge****ls are stimulated. One represents the cl****is, another the va**na and the third represents the cervix.

However, all three areas light up when the ni**le is stimulated.
This means the breast doubles as a truly sexual organ.
Therefore diseases of the breast, benign and more importantly malignant (that is breast cancer), really threatens ones quality of life.
