As a vascular surgeon, I've seen firsthand how endovascular techniques transform outcomes for patients with complete occlusions of the superficial femoral artery (SFA). Angioplasty—often subintimal or with adjuncts like drug-coated balloons—offers high technical success rates (up to 95-100%) for SFA chronic total occlusions (CTOs), especially in critical limb ischemia (CLI).
Recent studies show remarkable improvements post-procedure: ABI rising from ~0.21 to 0.82, rest pain dropping from 60% to 20%, and 80% radiological resolution of occlusions. It's a minimally invasive first-line option over bypass for many high-risk cases, promoting limb salvage and faster recovery.
