Thoracic endometriosis is a often underdiagnosed form of endometriosis in which endometrial-like tissue grows within the chest cavity, most commonly on or around the diaphragm, lungs, or pleura. It can cause cyclical symptoms that correspond to the menstrual cycle, such as chest pain, shoulder pain (often right-sided), shortness of breath, coughing up blood, or even lung collapse (catamenial pneumothorax).

Management of thoracic endometriosis requires a multidisciplinary approach, involving collaboration between gynaecological and thoracic surgeons. Laparoscopic and video-assisted thoracoscopic surgery techniques allow both pelvic and thoracic disease to be treated during a single surgical session. The goal is complete excision of visible lesions, repair of diaphragmatic defects, and prevention of recurrence.

Case Report: Lisa’s Journey
Lisa, from Ireland, travelled to our Endometriosis Centre for surgery. During evaluation, Lisa was diagnosed with extensive pelvic and severe thoracic endometriosis, affecting both sides of her diaphragm; surgery was performed in multidisciplinary team: gynecologist, colorectal surgeon, and thoracic surgeon.

Endometriosis can look very different from one person to another. For some, it comes with pain so intense that every menstrual cycle feels like a battle, days lost to cramping, nausea, exhaustion, and a body that refuses to cooperate. The discomfort can spill into every part of life, from work to relationships, leaving a quiet shadow over daily routines.

For others, it hides in silence. Cycles seem regular, pain is minimal or absent, and life moves on as usual. Yet beneath that calm surface, the same disease can still be present, small lesions, silent inflammation, slow changes that go unnoticed for years.

🇬🇷 Η ενδομητρίωση δεν δίνει πάντα «ορατά σημάδια». Πολλές γυναίκες μπορεί να φαίνονται υγιείς εξωτερικά, ενώ ζουν με χρόνιο και έντονο πόνο που συχνά παραμένει αόρατος για τους γύρω τους. Γι’ αυτό είναι σημαντικό να θυμόμαστε ότι η εικόνα δεν αντικατοπτρίζει πάντα την πραγματικότητα.
