CISTI E FISTOLE PILONIDALI
NUOVO TRATTAMENTO PER IL SINUS PILONIDALIS: E.P.Si.T
Tale tecnica è stata introdotta recentemente prima per il trattamento endoscopico delle fistole a**li (VAAFT) e poi tale la stessa tecnica e la stessa apparecchiatura sono state utilizzat eper la terapia del sinus pilonidalis.
La metodica è una tecnica mini-invasiva che utilizza un piccolo fistuloscopio di 3,2 x 4,8 mm (piccola sonda con un ca**le ottico ed un ca**le operativo e di irrigazione al cui interno vengono inseriti degli strumenti operativi :brush e bisturi endoscopico).
La tecnica è del tutto innovativa perché prevede l’introduzione di tale strumento all’interno dell’orifizio cutaneo che localizza la presenza del sinus pilonidalis e prevede due fasi:la prima fase diagnostica che serve per identificare la presenza del sinus ed eventuali tramiti fistoloso o cavità ascessuali secondari ed una parte operativa che prevede la perfetta pulizia di tutto il tramite fistoloso, cisti ed ascessi dall’interno ripulendo tutto il tessuto sporco ed infetto, asportazione dei bulbi piliferi che vi sono contenuti e la diatermocoagulazione di tale tessuto tutto sotto visione. L’intervento puo’ essere eseguito in anestesia locale o loco-regionale in regime di Day Sugery.

A**l fissure is caused by continuous contraction of the internal a**l sphincter. Internal sphincter spasm is the main cause of fissures. In case that nonoperative treatments fail to cure an a**l fissure, surgical treatment becomes mandatory.
