The role of the urologist/andrologist in the ICSI procedure

In the ICSI procedure (intra-cytoplasmic s***m injection), the urologist 👨‍⚕️ with expertise in andrology plays an essential role in identifying and managing the causes of male infertility.

📋 Patient evaluation is the first step. The urologist recommends detailed investigations to determine if there are s***m in the ej*****te or if an intervention is necessary to harvest them. These include hormonal analyses 🧪, testicular ultrasound and sometimes genetic investigations 🧬.

👇Er****on and male fertility – two different, but often confused aspects

Within the andrological practice, we frequently encounter men who associate the ability to have an er****on with fertility. It is important to emphasize that er****on and fertility are two distinct functions, regulated by different mechanisms.

✔️ Er****on is a vascular and neurogenic phenomenon, influenced by hormonal, psychological factors and cardiovascular health

✔️ Male fertility depends mainly on the s***m quality – concentration, motility and normal morphology of the s***matozoa

‼️ Thus, a man can have an active and normal s*x life, but still be infertile. Likewise, a man with erectile dysfunction can have normal s***matogenesis and become a father through natural or assisted methods.

📊 Recent studies show that approximately 15% of the infertile couples are faced with a cause of male origin, and of this percentage, a significant part have abnormal s***mograms despite intact s*xual function.

🔬 The evaluation of male fertility requires specific analyses: s***mogram, s***m culture, hormonal tests and, sometimes, genetic or imaging investigations.

Has male fertility declined globally or are we just diagnosing it better now?

🧪 The answer is supported by scientific data: male fertility is in a real, measurable and worrying decline.

📉 A study published in 2022 (Levine et al.) analyzed data from over 50 countries and concluded that:

➡️ S***m concentration (s***m count/ml) decreased by over 50% between 1973 and 2018
➡️ The rate of decline accelerated after 2000

⚠️ This phenomenon is not just due to advances in diagnosis. We are dealing with a real decrease in s***m parameters: total count, motility, morphology.

Chronic Stress and Male Fertility: A Scientifically Proven Link ⚠️

Prolonged psychological stress has a significant impact on male reproductive function. Studies show that men exposed to chronic stress can experience low testosterone levels, decreased libido, and significant alterations in s***m quality. 🧬

Teratozoos***mia and male infertility

Teratozoos***mia is a condition characterized by an increased percentage of s***m with abnormal morphology. According to the criteria of the World Health Organization (WHO), a normal percentage of s***m with typical shapes should exceed 4%. The presence of a significant proportion with abnormal shape can affect the fertilization of the oocyte and, implicitly, the chances of achieving a pregnancy.

🔬 Possible causes:
The etiology of teratozoos***mia is multifactorial and may include:
- genetic factors (chromosomal abnormalities, mutations of genes involved in s***matogenesis)
- hormonal disorders (testosterone deficiency, hyperprolactinemia, hypothalamic-pituitary-testicular axis dysfunction)
- chronic genitourinary infections
- varicocele (one of the most common correctable causes)
- exposure to toxins (smoking, alcohol, drugs, pesticides, radiation, elevated testicular temperatures)
- lifestyle factors (unbalanced diet, oxidative stress, lack of sleep)

📈 Evaluation and diagnosis:

Polys***mia is an increase in the ej*****te volume above 5.5 ml, a rarer condition, but which can have multiple causes.

The seminal vesicles and the ejaculatory ducts are important components of the male reproductive system, having essential functions in the process of formation and elimination of seminal fluid.

🔬 The seminal vesicles are paired glands, located posterior to the urinary bladder, which contribute significantly to the total volume of the semen. They secrete an alkaline fluid, rich in fructose, prostaglandins and other substances that support the mobility and the viability of s***m.
