【 #港怡健康教室：擊退子宮頸癌威脅】
子宮頸癌係香港十大女性常見癌症之一。為提升公眾關注，世界衛生組織將每年11月17日定為「世界子宮頸癌消除日」，呼籲女性定期接受篩查，減低患病風險。今次就等我哋簡單講解一下子宮頸癌嘅成因、篩查方法同預防措施。

人類乳頭瘤病毒（HPV）係主因
子宮頸癌主要由「高風險類型人類乳頭瘤病毒」（hr-HPV）持續感染引起。病毒主要經由性接觸傳播，部分人可自行清除病毒。不過，如果持續感染，子宮頸細胞可能出現病變，最終演變成癌症。

子宮頸癌高風險因素包括：
- 過早開始性行為
- 有多位性伴侶
- 多次懷孕或第一次懷孕時年紀較輕
- 長期服食避孕丸（超過5年）
- 曾感染性病
- 吸煙
- 免疫力弱

定期篩查至關重要
子宮頸癌愈早發現，治癒機會愈高。建議所有25至64歲曾有性經驗嘅婦女，無論有無接種子宮頸癌疫苗、已婚或未婚、停經、結紮或多年無性行為，都應該定期接受篩查。

常用篩查方法包括：
- 子宮頸細胞抹片檢驗
- HPV病毒基因檢測
透過採集子宮頸表面細胞樣本，可檢查是否出現異變或感染HPV病毒。

接種HPV疫苗係有效預防方法
除咗安全性行為，接種HPV疫苗亦係預防感染嘅重要方法。HPV不但可引致子宮頸癌，仲有機會導致生殖器官濕疣及肛門癌等疾病，所以無論男女都應該接種疫苗。

接種建議如下：
- 9至14歲人士：接種兩劑（第0、第5至13個月接種）
- 15歲或以上人士：接種三劑（第0、2、6個月接種）

【 : Beating Cervical Cancer】
Cervical cancer is one of the top 10 most common cancers among women in Hong Kong. To raise awareness, the World Health Organization has designated 17 November as World Cervical Cancer Elimination Day, encouraging women to undergo regular screening to reduce their risk.
Let’s explore its causes, how screening works, and what preventive measures you can take to protect yourself.

Human Papillomavirus (HPV) is the Primary Cause
Cervical cancer is mainly caused by a persistent infection with high-risk types of human papillomavirus (hr-HPV). The virus is primarily transmitted through s*xual contact. While some people can clear the virus naturally, a persistent infection can lead to abnormal cell changes in the cervix, which can eventually develop into cancer.

High-Risk Factors for Cervical Cancer:
• Early onset of s*xual activity
• Multiple s*xual partners
• Multiple pregnancies, or early age at first pregnancy
• Long-term use of oral contraceptives (over 5 years)
• History of s*xually transmitted infections
• Smoking
• Weakened immune system

Regular Screening is Crucial
The earlier cervical cancer is detected, the higher the chance of successful treatment. All women aged 25 to 64 who have ever been s*xually active—regardless of HPV vaccination status, marital status, menopause, or tubal ligation—should undergo regular screening.

Common screening options include:
• Liquid-based Pap Test
• HPV DNA Test
These tests involve collecting cell samples from the cervix to check for abnormalities and HPV infection.

Get vaccinated for prevention
In addition to safe s*x practices, HPV vaccination is a key preventive measure. HPV can cause not only cervical cancer but also ge***al warts and a**l cancer, so vaccination is recommended for both men and women.
Recommended dosage:
• Ages 9–14: 2 doses (at month 0 and between month 5-13)
• Ages 15+: 3 doses (at month 0, 2 and 6)

【專欄文章】TOPick — 醫語怡情︰肺癌治療新里程　內窺鏡無創結節消融術

肺結節是腫瘤嗎？肺部出現結節就是患上癌症嗎？肺結節是指在肺部出現、直徑通常小於三厘米的陰影。事實上，大部分肺結節屬良性，一般而言，肺部出現結節不等於是患癌，醫生需根據結節的大小、形態等，以判斷其演變成惡性腫瘤的風險。

肺結節大多沒有明顯症狀，通常於身體檢查時被發現，成因包括感染後疤痕、肺炎、積痰、免疫系統引致炎症、良性腫瘤等。若患者長期吸煙或有肺癌家族史等風險因素，則需加多留意及定期監察肺結節狀況。

隨着電腦掃描技術進步，透過清晰影像可發現微細結節，並能分辨出其不同狀態。肺結節可分為實心結節、磨砂玻璃狀結節及混合型結節。根據國際指引，若發現直徑超過六毫米的結節，醫生會視乎病人本身的風險因素，評估其需否定期觀察及檢查頻率；如磨砂玻璃結節隨時間變得實心、邊緣不規則、直徑大於一厘米或有明顯增大，結節演變為腫瘤的風險較高，建議抽取組織化驗。

如確診早期肺癌，切除手術和放射治療仍是主要方案，但手術具創傷性，需要切除部分肺葉，電療則可能影響周邊健康組織。若患者的身體狀況不適合接受外科手術，可考慮採用微創的消融治療，例如利用射頻、微波、激光等技術，產生熱能來破壞腫瘤細胞。治療過程中，醫生會經皮膚穿刺，將探針或導管精準地引導至腫瘤位置，並將高能量傳遞至目標區域，以達到消融癌細胞的效果。由於治療集中於腫瘤範圍，能保留更多肺功能。然而，以穿刺方式進行治療，可能導致氣胸及出血等併發症的風險約為 25%至30%。

隨着近年電磁導航支氣管鏡技術的發展，現時可經氣管以無創方式進行活檢及消融，創傷及併發症風險亦能降低。系統利用電腦掃描影像可建立支氣管結構的三維圖像，並導航支氣管鏡準確地到直達病灶，甚至可深入肺部縱橫交錯的分支位置進行消融治療。整個過程毋須任何切口，病人最快可於手術翌日出院，氣胸及流血等併發症風險可下降至約2%至3%。

對於年長、有哮喘或肺氣腫等未必適合接受傳統手術的患者，新技術能保留肺功能，同時清除病灶，亦為多發性原位癌患者提供多一個治療選項；針對擴散性腫瘤，此技術更可配合免疫治療，提高療效。

【世界肺癌關注月】
肺癌一直高踞香港致命癌症首位，吸煙係導致肺癌嘅主因，醫學臨床研究顯示，約九成男性肺癌患者都有吸煙習慣。由於早期肺癌通常無明顯症狀，超過一半患者確診時已屬晚期，因此定期篩查係好重要，先可以及早發現、及早治療。每年11月係「世界肺癌關注月」，等我哋一齊提升對肺癌嘅認識，守護肺部健康！

🔍 兩大類肺癌類型
-小細胞肺癌（約15%）：與吸煙有密切關聯，病情發展迅速。
-非小細胞肺癌（逾80%）：主要包括肺腺癌、鱗狀細胞癌同大細胞癌。其中以肺腺癌最常見，較常見於非吸煙人士同女性。所以，即使從未吸煙，亦有可能患上肺癌。

🩺 常見肺癌診斷方式包括：
-低輻射劑量肺部電腦斷層掃描
-胸部X光檢查
-痰液檢測

【World Lung Cancer Awareness Month】
Lung cancer has consistently ranked as the leading cause of cancer-related deaths in Hong Kong. Smoking is the primary risk factor, and clinical studies show that around 90% of male lung cancer patients have a history of smoking. As early-stage lung cancer often presents no obvious symptoms, more than half of patients are diagnosed at an advanced stage, making regular screening critically important for early detection and timely treatment. November is World Lung Cancer Awareness Month – let’s work together to raise awareness and take proactive steps to protect our lung health.

🔍 Two major types of lung cancer
-Small cell lung cancer (about 15%): Closely associated with smoking and progresses rapidly.
-Non-small cell lung cancer (over 80%): This category mainly includes adenocarcinoma, squamous cell carcinoma, and large cell carcinoma. Among these, adenocarcinoma is the most common type and is more frequently seen in non-smokers and women, highlighting that even non-smokers can be at risk.

🩺 Common diagnostic methods include:
-Low-dose CT lung scan
-Chest X-ray
-Sputum a**lysis
