The Care Team is seeking a Client Care Supervisor for a community-based role in South Dublin.

The ideal candidate is highly organized, flexible, proactive, and possesses strong time management and communication skills.
This is a hands-on supervisory position responsible for overseeing daily care operations and ensuring the highest quality of service delivery.

🔑Key Responsibilities:

✔ Case management of community-based clients: assess new clients, develop care plans, and support client independence.
✔ Supervise, train, and support healthcare assistants in the field.
✔ Conduct client and caregiver assessments, introduce caregivers to clients, and ensure care plan adherence.
✔ Perform quality checks to ensure high standards of care, safety, and professionalism.
✔ Manage changes in care plans, rosters, and team schedules.
✔ Ensure adequate coverage for absences and holidays.
✔ Collaborate with the rostering team and line manager to meet operational needs.
✔ Maintain accurate client records and participate in on-call rotations.
✔ Assist the Line Manager with administrative and support tasks.

👉Requirements:

✔ FETAC or QQI Level 5 in Healthcare (or equivalent).
✔ Minimum of 1–2 years in a supervisory role or 2+ years as a Healthcare Assistant.
✔ Proficient in Microsoft Office and Outlook.
✔ Strong communication skills.
✔ Full clean Irish/EU driving licence (own car is optional).

This is a dual-function role combining field-based client assessments with live-in care coordination.
You’ll be responsible for developing personalised care plans, ensuring compliance with HSE standards, and supporting a team of dedicated live-in carers across the region.

🛠️ What You'll Do:
Client Assessment Responsibilities:

• Conduct comprehensive in-home or virtual client assessments
• Create person-centred care plans and perform risk assessments
• Liaise with families, GPs, nurses, and allied health professionals
• Maintain accurate documentation and submit monthly KPI reports to the HSE
• Participate in the on-call rota
Live-In Care Coordination:
• Manage live-in care packages and build rotas for carer coverage
• Provide onboarding, training reminders, and ongoing support for carers
• Ensure service continuity and high levels of client satisfaction
• Monitor compliance with safeguarding, GDPR, and HSE requirements
