Our bodies have an amazing ability to heal after injury.

Pain will decrease, movement will improve and before long you’ll be back walking as good as before. 

However, one of the big pitfalls that many people can make when recovering from pain or injury is thinking ‘are sure the pain is gone, I’m walking fine, I must be ready to go back to the gym again.’

This ‘pitfall’ can lead to 2 problems.  

1 - The same area gets reinjured. 

2 - A new problem develops elsewhere.

To prevent this from happening to you, and anyone you know (spread the word) , ask yourself this

‘have I earned the right to go back to ….. Insert X,Y,Z 

For example, have I earned the right to go back running, 

lifting weights….. My fitness class …..etc

Because while the pain will nearly always settle with time, this does not mean that the strength or mobility is there to be able to do what you want to do.

In this video I give a simple way to help you figure out how to ‘earn the right’

That is by working backwards from what it is that you want to get back doing.

A lot of people can get stuck in a limbo or been afraid to go back to the things they enjoy for fear of hurting themselves again, 

Or getting stuck in a cycle of feeling good, getting injured, feeling good and so on

Having an MRI for your back can be scary,

Getting the report can often be worse

However, despite all the worrying sounding names and multiple findings that you will find in your report, a lot of them can be normal signs of wear and tear and may not be a factor in your back pain. 7 

So in this video I’ll be breaking down the most  common findings in MRI reports to help you understand what they mean to you and your back pain.

Back pain can be confusing.

When it gets bad, your instinct might be to lie down and rest…
Or you might wonder if you should try to “walk it off.”

The truth is — it can depend.

Back in the day people were often advised to rest in bed until the pain settled, however evidence showed that this led to fear of movement, deconditioning, more sick days off work and worse long term outcomes.

The best practice nowadays is to encourage appropriate movement whenever possible to try and relax the nervous system, reduce the pain and speed up the recovery.

However, that can often be easier said than done if you are in a lot of pain and not all back pain will respond the same way.

So, here is my advice.

➡️See what movements you can do.

➡️Try to relax your body and breathe as you do them.

➡️Only move within a pain free range and importantly, don’t try to push through the pain.

➡️And if any positions are sharp and very painful to do then try to avoid them until the pain settles.

There are some good clinics out there, but there are plenty of mediocre ones too.

To help you decide if my clinic is the right one for you, here is a little flavour of how you will be treated at my clinic.

1 - Take the time to listen
You wouldn’t believe how many stories I have heard of clinicians talking at people and NOT ACTUALLY listening to what they have to say

2 - Help you understand the root cause.
It’s very easy to find the painful area and focus on that. But more often than not the painful area is a reaction to an issue elsewhere in the body.

In order to get LASTING change this needs to be identified and addressed.

3 - A treatment plan

Every journey needs a plan. Once I understand what your goals and what you need and want to do, I will map out a plan to help get you there.

4 - Honest and upfront

I’m not a magician. Overcoming pain and injury can take time and a commitment. I’ll let you know what that entails.

5. Lasting solution

Panadol can relieve pain. But it can’t help you build the strength and mobility needed to help you confidently do the things you love, for as many years as you are able to.

Pins and needles, pain travelling down your arm. 

What should you do? Who should you see?

In the video I explain how I was able to help a patient in my clinic who had these symptoms. 

Pain is complex and there are times when I am not sure what is causing the pain when someone comes to see me.  

But so long as I am happy that there is nothing serious going on that needs medical intervention, I believe that there are a number of things that can be done to help someone feel better and move more easily. 

And rather than doing nothing and holding out for an MRI or waiting for the painkiller or injection to work, you can be actively doing something to try and help reduce your symptoms. 

This is what I did with a patient recently who had a bad episode of neck pain and nerve pain down his arm. 

And it helped a lot.

Don’t overlook this when it comes to overcoming back pain. 

It is one of the first things I will focus on with my patients in my clinic.

So many people are under the impression that they need to tighten their core, tuck their belly in and sit up straight and upright, 

All this does is encourages tension

To a back that is already tight and tense.

So, if you have been struggling to overcome back pain, you are fed up and frustrated because you have been following your physio’s advice but your not improving,

Then it’s time to seek out a different approach.

Let me tell you how I used an iceberg to help Paula finally overcome her back pain for good.

And this after 2 years of trying a mix of different things that only gave short term relief. 

When Paula came to see me for her first session, I listened to her story of how her pain came about and even before assessing her movements, I had a good idea of what was happening. 

So I drew an iceberg and began to explain that the painful area, which was her low back, wasn’t her problem area. 

And she had spent the last two areas focusing on addressing the tip of the iceberg and not the main bulk of it underneath.

The main problem area for Paula happened to be a mixture of a weakness in her right leg that stemmed from a calf injury, 

Poor advice that she had received over the past 2 years which had to her been overly protective of her back and afraid to move, 

And this led to poor daily movement habits which were causing her back to get overloaded.  

And while she had gotten some relief from addressing the 20% of the iceberg, which was her low back, her pain kept coming back because she hadn’t addressed the 80%, which was the rest of things I mentioned above.

Once we set about addressing the 80%, her back pain went away and she was able to return to enjoying exercise again, without her back pain returning.

If ice, rest and waiting for the pain to go away is your solution to a throbbing knee after running then let me tell you…..

That I have done this before in order to keep running and it will only ever be a SHORT TERM fix and your knee pain WILL return.

Fortunately I learned from my own mistake and over the last 10 years as a physio helping tonnes or runners I now have a much better approach for runners over the age of 30.

When someone comes to see me with knee pain there is often one vital thing that they are missing.

Because it plays such a vital role in overcoming knee pain, it is usually the first thing I look to address with my knee pain patients. 

What is it?

It is how the muscles below and above the knee interact with each other.

Let me briefly explain.⬇️

When you walk, run, get out of a chair or lunge to pick something, there is a force that comes up from the ground that your body needs to be able to absorb.

It starts at the foot and goes through the ankle, knee, hip, pelvis, core and the rest of your body.

Each muscle along the way has to interact with the muscle below and above to absorb this tension and pass it on to the next area. 

This is how we move in a controlled and efficient way. 🙂

When it comes to knee pain clients I see, I commonly find that the muscles below the knee (and calf and foot muscles) and the muscles above the knee (hamstring, quads and glutes) are not working effectively together. 

Which means that excessive tension can be absorbed by the knee joint instead of being absorbed by the muscles. 

Even if you rest your knee, take pain killers or do all the best stretches for your knee, there is a strong chance that your knee pain will return unless you rectify this problem. 

How do you rectify it?

In my clinic I follow this approach,

👉identify the cause of the problem,

👉Figure out what needs to happen to resolve the problem.

👉Put a plan in place and help my patient through this plan.

Hi, it’s Derek here, physio and clinic owner at Midlands Physio & Back Pain Clinic. My clinic is based in Moate, Co. Westmeath and our aim is to people get back to doing the things they love and need to do, without pains, aches or stiffness getting in the way. We are a small clinic but we put a big focus on finding the true ‘stressor’ or cause of a problem in order to get long lasting results and not just short term pain relief.
