خلع الكوع مع الكسر من أكثر الإصابات تعقيدًا في مفصل الكوع، خصوصًا عند وجود كسر في الزاوية الأمامية لعظمة الزند (Coronoid process) وتمزّق في الرباط الجانبي (LCL).

في هذا الأسبوع قمنا بعلاج إصابة كسر (Coronoid fracture) بطريقة منظار الكوع (Arthroscopic-assisted fixation) باستخدام K-wires لتثبيت الكسر بشكل دقيق مع أقل ضرر للأنسجة، إضافةً إلى إصلاح الرباط الجانبي (LCL) بواسطة Anchor لاستعادة ثبات المفصل.

هذا النوع من العمليات يتطلب فهمًا عميقًا لميكانيكية مفصل الكوع وتقنيات ترميم الأربطة والحفاظ على الاستقرار الخلفي-الجانبي.

الحمد لله، المريض بخير، والمفصل مستقر، وسيبدأ برنامج الحركة المبكرة.

Elbow fracture–dislocation is one of the most challenging injuries around the elbow, especially when it involves a coronoid process fracture and disruption of the lateral collateral ligament (LCL).

This week, we performed an arthroscopic-assisted fixation of the coronoid fracture using K-wires to achieve precise reduction with minimal soft-tissue trauma. In addition, I repaired the LCL using an anchor, restoring posterolateral stability of the elbow.

This procedure requires advanced understanding of elbow biomechanics and meticulous ligament repair to ensure a stable and functional joint.

Thankfully, the patient is doing well, the elbow is stable, and early motion is recommended!

Our goal is always to help patients return to their normal life with the least invasive and most effective approach.

"الاتجاهات الوبائية والنتائج المرتبطة بالعمر في عمليات مفصل الكتف العكسي الكلي: تحليل وطني أمريكي يشمل 9692 مريضاً"

🔍 تسلط الدراسة الضوء على الفروقات العمرية في أسباب العمليات ومضاعفاتها بين الفئات العمرية المختلفة.

💡 هذا التحليل هو من أكبر الدراسات التي تناولت هذا الموضوع باستخدام قاعدة البيانات الأمريكية، ويؤكد على أهمية إدارة ما قبل وبعد العملية بشكل مخصص لكل فئة عمرية لتحقيق أفضل النتائج.

تثبيت بعض كسور الكوع بالمنظار الجراحي أصبح خياراً فعالاً ودقيقاً في علاج الكسور المعقدة حول المرفق. هذه التقنية تسمح برؤية مباشرة لمكان الكسر، تثبيت دقيق، وتقليل الضرر على الأنسجة ، مما يساعد المريض على العودة السريعة للحركة.

نعلن بفخر عن قبول ونشر بحثنا الجديد في مجلة Surgical and Radiologic Anatomy :
تتناول هذه الدراسة العلاقة الدقيقة بين العصب PIN وبين البنى التشريحية القريبة من الحدبة الكعبرية، بهدف تحديد المناطق الآمنة أثناء إصلاح وتر العضلة ذات الرأسين.
أظهرت النتائج أن وضع الساعد في حالة الاستلقاء (supination) يزيد المسافة بين العصب والحدبة الكعبرية، مما يقلل من خطر إصابة العصب أثناء الجراحة.

Proud to share our newly accepted publication in Surgical and Radiologic Anatomy titled:
“Anatomical considerations for safe retractor placement in distal biceps repair: a cadaveric study of the posterior interosseous nerve.”

This anatomical cadaveric study defines safe surgical zones around the radial tuberosity during distal biceps tendon repair.
Our findings highlight that forearm supination increases the distance between the posterior interosseous nerve and the radial tuberosity, thereby reducing the risk of iatrogenic nerve injury.

تؤدي إلى ضعف في بسط المرفق وصعوبة في أداء الحركات اليومية.

أجرينا إصلاحًا جراحيًا باستخدام أنفاق عظمية (Transosseous Tunnels) لاستعادة القوة، الثبات، والحركة الطبيعية للمرفق.
تذكير مهم: ألم المرفق الخلفي بعد إصابة يجب أن يُثير دومًا الشك في إصابة وتر العضلة ثلاثية الرؤوس.

🔹 Triceps tendon tear is uncommon but often overlooked.
It causes weakness and loss of elbow extension, limiting daily function.

We performed surgical repair utilizing transosseous tunnels to restore strength, stability, and motion of the elbow.
A good reminder: posterior elbow pain after trauma should always raise suspicion for a triceps injury.

كان لقاءً علميًا مميزًا وفرصة رائعة لتبادل الخبرات ومناقشة أحدث التقنيات في جراحة الكتف والمرفق.

It was an honor to participate as a faculty member in the Arabic Shoulder and Elbow Conference, bringing together top surgeons and experts from across the Arab world.
It was a remarkable scientific meeting and a great opportunity to exchange experiences and discuss the latest advancements in shoulder and elbow surgery.

قمنا مؤخراً بعلاج مريضة تبلغ من العمر ٧٤ عاماً، نشيطة وبصحة جيدة بشكل عام بعد أن تعرضت إلى كسر معقّد بأربع أجزاء في عظم العضد.

استخدمنا تقنية PALMS مع تثبيت (Greater Tuberosity) بالخيوط المتينة وتثبيت بالسيركلاج، مما ساعدنا على إعادة بناء التشريح وتوفير ثبات ممتاز للكسر.

💪 هذه الحالة تذكير بأن التخطيط الصحيح ومزج القديم بالجديد يمكن أن تمنح حتى المرضى المتقدمين في العمر فرصة للعودة إلى نشاطهم واستقلاليتهم.

We recently treated a 74-year-old active and generally healthy patient who sustained a complex four-part proximal humerus fracture.

We applied the PALMS technique along with greater tuberosity (GT) suture fixation and cerclage, which enabled us to restore the anatomy and achieve solid fixation.

أسبوع مميز قضيته في المشاركة بدورة تدريبية على جثث (Cadaveric Course) وفي فترة تدريبية (Rotation) في مستشفى Regions Hospital في الولايات المتحدة الأمريكية ،حيث ركزنا على كسور لوحة الكتف المعقدة وإصابات جدار الصدر.

هذه التجربة الغنية أضافت إلى معرفتي وساهمت في توسيع فهمي للإصابات النادرة والمعقدة.

A special week participating in a Cadaveric Course and a clinical rotation at Regions Hospital, in the USA. focusing on complex scapula fractures and chest wall injuries.

This valuable experience enriched my knowledge and expanded my understanding of rare and complex injuries.

🔍 ما هو انضغاط العصب فوق الكتف؟
يمر العصب فوق الكتف عبر ممر صغير في أعلى عظمة الكتف. أحيانًا يتعرض هذا العصب للضغط بسبب رباط مشدود، كيس، مما يسبب ألمًا في الكتف، ضعفًا في القوة، وضمورًا في العضلات.

🔍 What is Suprascapular Nerve Impingement?
The suprascapular nerve passes through a small tunnel at the top of the shoulder blade called the suprascapular notch. Sometimes, this nerve can be compressed by a tight ligament, cyst, or scar tissue — causing shoulder pain, weakness, and muscle wasting.

💡 Arthroscopic Release is a modern, minimally invasive procedure where we use a camera and fine instruments to cut the tight ligament and free the nerve. This helps relieve pain, restore strength, and prevent further muscle damage — all through tiny incisions and a faster recovery.
